Mysterious shortness of breath: What has helped you?

I will try to make this as short as possible, but this has been going on for over 5 years, so it might be farily long. 

Beginning in summer of 2012, I began having shortness of breath (SOB) with no other symptoms. I felt a constant need to yawn, and every few breaths wouldn't satisfy the SOB. I would take a deep breath, and felt like it would get "stuck" before satisfying the air hunger feeling. About every 3-5 deep breaths would satisfy it, only for it to return a minute later. 

I got an endoscopy and other tests done, which revealed that I had some esophageal erosion due to acid reflux and a slight hiatal hernia and was diagnosed with GERD. I had always have bad heartburn, so I was prescribed with Prilosec, which I have been taking daily since them. I've tried stopping it a few times but the reflux always comes back a lot worse. 

Lung tests and x-rays were normal. Heart tests normal. Blood test revealed a slight anemia but otherwise pretty normal. 

I did some research reading forums where someone suggested taking vitamin B-12. Strangely, I took it and the SOB disappeared almost instantly. However, it only lasted a few days for it to return just as bad. I then started taking an iron supplement, which again made the SOB disappear quickly- same thing; symptom returned days later. 

After further research, I came across a breathing exercise method called the Buteyko method. Essentially you do a lot of breath holding to build up CO2 and reduce breathing as the theory is that I had chronic hyperventilation causing too much CO2 to exit my body. After applying the method and reducing my breathing, the SOB disappeared after only 2 days and I felt completely normal. I continued the method a few more days then no longer felt the need to pursue the exercises. I was normal for a whole year when the SOB once again returned with some chest tightness. I applied the method again and the symptom went away, this time with a little more effort; after about 3 weeks. I included physical exercise which also helped with my breathing. 

After that, I was normal for about 2 years. I mistakenly stopped or at least slowed down exercise and the SOB returned once again. I applied the method and began running for exercise but the SOB kept getting worse. It got so bad, I had multiple panic attacks and the feeling of completely empty lungs with the inability to satisfy it with deep breaths. I had to stop exercise altogether, apply the Buteyko method and do breathing exercises very carefully with very light and slow exercise. This helped, but it took many weeks for the SOB to improve. Then, it was almost normal when over a year ago as I was running, I couldn't get a deep breath to satisfy exercise-induced SOB. I have had SOB continuously since then (a year and a half). 

I once again started doing breathing exercises and slowly building up physical exercise, but I can't do any prolonged cardio activity because the SOB gets to a point where deep breathing will not satisfy it. While the breathing exercises have helped, they have had very little effect compared to previous efforts. It seems that every time the symptom returned, greater effort yields few results.

I suspect there is something, some underlying cause that is causing the SOB that has alluded me this entire time. 

So for the past few months to a year, the SOB is worse on some days, better on others, but never gone. There's no rhyme or reason or pattern for it. It's just there, sometimes affecting my sleep. I sometimes can't get a deep breath to satisfy it every now and then, but for the most part, a big gulp of air will satisfy it. But it returns seconds to minutes later. It's as though every breath doesn't deliver what it's supposed to, the SOB builds up, and then I have to take a big gulp of air to get rid of the feeling, pattern repeats. My breathing pattern is normal, however. I don't feel like anything physical is happening, but sometimes it feels like my airways and nostrils are slightly inflamed due to allergies, but when I don't feel inflammation the SOB is still there. 

Recent lung function tests show normal- I don't have asthma, or any other problems with my lungs. Heart tests are normal though I did have about a two week bout of heart palpitations which came and went. Haven't had any for a while- it just mysteriously started happening then stopped. Blood tests are normal, though tests always show a slight elevation of biliruben which my doc thinks is Gilbert's disease. 

I don't have sleep apnea (normal test), bloody oxygenation is normal, heart rate normal. 

I recently saw local naturopath (since mainstream docs aren't able to help) who immediately suspected a liver problem when I described my SOB, possibly liver inflammation. He used an electrodermal testing machine to test his theory which did seem to show a problem with my liver and gallbladder. He gave me digestive enzymes and a gallbladder formula to help clear a bile duct clog, thus reducing liver inflammation. He also determined with the machine that I have an egg sensitivity so I've been avoiding eggs. 

Been taking this and avoiding eggs for a couple of months, but there has been no noticeable improvement. Everything else is normal. Emotionally I'm normal- no anxiety, depression, etc. The SOB seems to be the only symptom of something, but always comes back worse, until a year and half ago when it came back and has remained since. I feel like I shouldn't have to do breath holding exercises every day just to maintain my breathing well enough to do every day things. 

Does anyone have any idea of a possible underlying cause?

2021/2022 UPDATE:
Since my original post about 3-1/2 years ago, a lot has happened, so I’d like to update the post to share with others who come across this what I’ve done since then.

I have maintained a weekly Buteyko method breathing exercise regimen where I do a few of these breathing exercises 3-4 times a week in the morning. This, in combination with daily light to moderate exercise (specifically weight lifting, with 1 or 2 days a week of walking and light jogging), I feel has kept the air hunger symptoms tolerable and manageable. I have mostly good days with some not-so-good days, but doing a breathing exercise and knowing that it’ll get better again helps me get through those times.

I also have sinus inflammation which can exacerbate the symptoms, but I’ve also managed this, which in turn reduces the severity of the air hunger symptoms.

So, while I haven’t found a cure nor is the problem completely gone, I have been able to maintain normalcy in daily life and manage the symptoms through the strategies I described.

This discussion remains active, alive and well through the comments section where others who have similar symptoms have shared what has helped them as well as suggestions for possible solutions to explore.

I have but I have been moving a little slower. I Just made it to Module 4. I am trying to find an Alexander Technique teacher and there are a few but it is kinda pricey to have sessions. But If it helps, I am willing to spend money on something that can drastically change my future! Keep me updated as well!!

I think the AT teacher may be a little expensive so I am trying do it without doing too many AT sessions but we will see.

But so far it makes so, so so much sense as to what I am experiencing! I am really hoping the technique gets incorporated into my life so I will finally have something I can do when experiencing bad breathing tension while exercising.

I am going to try working through it without an AT teacher first and see how it goes. With the money I have spent in ER visits and doctors and tests, $90 didn’t seem bad at all.
So far everything I am learning makes since with how I feel with my breathing.

Yeah same here! are you going to attend office hours this Saturday on Zoom? I would love to connect with you on this and see how you're progressing! and yeah everything makes sense with me so far as well! I am thinking when I was cutting pressure treated lumber, I had a panic attack and that's what set off my breathing interference. Anyway, hopefully others in this thread can find use as well! Of course, I am only arriving to this process after ruling out mostly everything else (I.e. VCD, GERD, Asthma, Heart issues, etc...).

And yeah, even with Insurance, I have spent close to $2000 on doctor visits in the last year, so yeah, $90 isn't bad and if I need to pay an additional $500-1000 for AT lessons in person, still worth it!

I just happened to read the digest today and this thread caught my eye...... I've been dealing with SOB with just basic activities in the house for over 3 yrs! finally, this past week I began oxygen 24/7 and what a diffetence!!! I've been on cpap (and am in the process to get the Inspire implanted device). PFT was even better from 3 yrs ago! the common link has been my pulse oximetry drops at rest, esp at night (low 80s). but there is no lung disease explanation!!! I recently bought a wellue 02 ring to wear at night to monitor things. all I know as long as I wear the 02 I'm not SOB with activity. I finally have more energy to ride my bike and walk the dog. hoping this will boost my metabolism to lose weight. and after my recent dental bone graft I hardly had any pain (for once!).

I've really had to advocate for myself..... grrrr

I’ve had this on and off too. With me it seems to either be a result of stress or diet as I have GERD too. I think one of the best things you can do is change your diet to eliminate the GERD naturally. The autoimmune paleo diet or paleo in general is a good place to start. Gods speed to you!

Hello, I hope this still reaches you. How are you now? I am miserable now in same situation as your post. I cannot find out why. Some differences tho. I have a wheeze but they say they can’t hear with stethoscope but you can hear with normal ear. I have a borderline mild obstruction from PFT. I had a lung tumor removed in April but this is secondary. All other tests are normal. I’m defeated and depressed. Nothing has made this go away. I also feel like

any surgery and anesthesia can result in depression too..... r u on an antidepressant (i take 2)? do u check ur pulse ox throughout the day? that can be reassuring OR reaffirming, that there is a reason for how u r feeling..... I also have asthma (only use nebulizer when having problems) and have had intermittent chest xrays showing atelectasis (since 2016)! no one ever told me! I also have Gerd and have been taking prilosec for years. even tho I'm a retired nurse, why can't I just be a patient and have the dr figure things out??? I feel ur frustration. hang in there, u r not alone!!!

Yes I have and it did help very much. However as of late I’m a little bit lax with my diet and still doing alright. Certain things like nightshades (tomatoes, peppers, etc.) really make my GERD flare up so I make a point to avoid those.

Thank you so much for posting. I’ve had an issue of not feeling like I got enough oxygen when I breathe deeply and all tests show normal. The desire to yawn a lot hit me. I run a group and hated yawning as they were talking but needed to. Eventually, I had to let them know I was struggling quite a bit. Anxiety clearly made it worse but I fainted playing tennis because I couldn’t get a breath and became dizzy 5 minutes into playing when I typically play 2 hours. Drs seem to think it’s anxiety, which I figured they would but I feel I am very aware when my anxiety is causing physical symptoms. I changed my diet completely and meditate daily as well as yoga. I feel like it’s all in my gut for some reason. X-rays show excessive gas but I had that issue since I was a toddler. I notice I breathe better laying down with something under my lower back.
Sorry for the rant, you just posted the closest thing I’ve heard to what I am experiencing. I’m hoping it’s something that isn’t just there. I’m a healthy 30 year old. It just feels like something that doesn’t have to be permanent.
