Do you have increased coughing at night?

Posted by caontm @caontm, Sep 4, 2022

Hi all, I tend to have more (dry) cough at night when I lay down even though during the day I do two 7% saline solution nebulizing and can cough up a lot of yellowish sputum and I think my lung should be clearer at night. Do you have simile experience or can help explain the cause of the night cough? The cough disrupts my sleep and that of my family members. Thanks!

My Pulmo RXd benzonotate, a capsule which is supposed to suppress the cough at pm. It’s not covered by my insurance, but it’s not expensive, about $35/month. I like this better than a cough syrup. But I don’t tend to cough at pm which was the plan so I could get a good nights rest. Please see what your pulmonologist says. I’m at the beginning stage of treatment for my Mac-Nt.

Benzonotate has been a welcome event for me. Still coughing up whatever is in my lungs is starting to choke me - very scary!

If I cough at night I take some expectorant syrup.
Guaifensin syrup loosens things and I stop coughing. I make sure it plain with no decongestant as that raises my blood pressure. Worth a try?

Sue, are you clear of NTM? Do you get bronchitis often and why?
Does wearing a mask when with others help in limiting the exacerbations?
I’m afraid of getting exposed to people other than my family members I live with. My biggest fear is catching a respiratory infection on top of MAC. I thank the pandemic for teaching us to wear masks and for people not to question.

As far as I know, if I still have MAC, it is subclinical. I quit antibiotics ww months ago while still positive, but lungs are stable. I see the doc next week. I have bronchiectasis, asthma, reactive airways and allergies, hence my susceptibility to bronchitis. I have lived with it for so long that I don't spend much time worrying about it, but do wear a mask quite often indoors. I go by a mental formula for how long, how close, and how much airflow. So I wear a mask in church where we are elbow to elbow for over an hour, at PT where I am in close proximity to the therapist for an hour, in a theater, in a crowded store. But not in wide open malls where I am passing through, at a drive through or during other incidental contact. We wear masks if our grandsons are ill and we care for them. Every exacerbation has come from a virus I picked up from my kids or grandkids. (Unmasked) Among people here I am probably less isolated than most, but most of our friends and family are very good about staying home and canceling social plans when the least bit ill.

Thanks for your reply. I ended up in the ER on Friday night because of the amount of wheezing and mucus I was experiencing. It was a tough night. I wasn't seen for about 4 hours after arriving , After bloodwork and an Xray, I was discharged about 10 hours with the xray being clear...no pneumonia and the bloodwork was ok.
I coughed and wheezed all night last night and brought up copious amounts of clear mucus. I see an ID doctor on WEdnesday and I have placed a call today to the pulmonologist. I have no appetite as I am afraid to eat that I will start coughing.
The ER doctor told me he could not give me a steroid since I have MAC lung and I am still on 3 antibiotics.
Wondering again if nebulizing hypertonic saline twice a day is creating too much mucus. I am beyond frustrated and can't believe that the way I wheeze and cough all day is part of bronchiectasis and MAC lung.
Thanks for listening to me. I just don't feel like I am being heard by doctors.

Did the ER doctors do a sputum test?
If not, get one done — even your primary care doc can order it.
I had similar experience a few weeks ago and ended up in hospital for three days then back again for four days — violent unusual coughing cracked a rib and pulled back muscles. Sputum showed I had Acinetobacter Baumanni and was put on IV antibiotics and steroids.

That is a good suggestion - are you doing better now?

Yes, I’m on the uphill — thank you.
But I’m dismayed at the havoc these coinfections can cause us — way more traumatic and serious than the MAC infection.
This is my second coinfection in three years — first one was staph aureus. Like you, I don’t want to live in a sterile bubble and I do take common sense precautions …, but geez……

I understand "I just don't feel like I am being heard by doctors." And I would guess that at least 80% of the people on this site have had the same feeling at one time or another.

The ER is the most frustrating place for me - but my daughter (formerly a paramedic & ER nurse) explains - "Our job is to stabilize people and move on, so rare diseases and infections are out of our realm. Most of the docs and nurses have never seen or heard of MAC, and may even try to tell people that they are on too many antibiotics." This is not an indictment of the system -simply an acknowledgement that every part of the medical "complex" has a role - theirs is to make sure we stay alive until we can see our own provider.

Perhaps you can discuss lessening the saline nebs - maybe go to 3%, or once a day? One question, since you mentioned that coughing can follow eating - have you been evaluated to see if you have GERD or a swallowing disorder?

I saw my pulmonologist yesterday. He put me on Ipatrium/Albuterol, Ceftin & prednisone and Singulair for allergies.
I am seeing an ID doctor today. Sputum culture was normal flora and the chest xray was clear. He did hear a great deal of wheezing.

Hi ReRe,
Taking steroids will increase the probability of infection. Of course, we walk a tightrope when trying to manage our symptoms and treat our disease. All the best, Linda Esposito

Hi Linda
Yes, I know but the wheezing and coughing has been uncontrollable with very little sleep at night. Since he prescribed inhaled Budesonide, I was able to get some sleep last night. My last 2 sputum cultures were negative for MAC lung.
