Lung mass lesion - need expert opinion
My mother was detected with a lung lesion measuring 3.3 *2.4 cms in right lung upper lobe posterior segment in May 2021 when she was covid positive (accidental finding). 15 day later, Later we had one more CECT scan, and the size came out to be 1.4 * 1.6 cms, then pet scan showed sized 2.5*2.7 cms (August 2021) with SUV max as 1.7.
Follow up ct scan in June 2022 showed size as 2.4*2.1 cms with SUV max as 1.4... and pet scan in June 2022 showed 3.2*2.5 cms. Everytime it showed minimally hypermetabolic lymph nodes and few non avid subcentimetric lung nodules.
the FNAC says its adenocarcinoma.
P.S. I am awaiting detailed biopsy results to visit the oncologists. Just wanted a quick check what all are our options for treatment.

@sahiltaneja - Good morning and welcome to Mayo Clinic Connect. It must be a tough time for you. When my mom was ill my heart broke.
Hypermetabolic lymph nodes were caused by your mom's immune system, most likely and probably from COVID. This means that they have swollen which makes sense. It could also be a combination of both cancer and COVID reaction. People who have had Covid vaccinations also get this reaction quite often. Non Avid lung nodules have to do with a reaction to an antigen which I simply do not understand.
As far as treatments for your mom it will depend on what the biopsy shows and it depends on what your mom wants to do. Adenocarcinomas can be treated quite easily now with quite a few options but it really does depend on her medical history, her other illnesses, if there are any, and other factors that would be considered for any operation.
How is your mom and and how is she reacting to her news of having lung cancer? And how are you?

I will wait for Biopsy results and update this thread. She is doing fine, no symptoms at all, just slight breathlessness for last 2 years. She just has high blood pressure, other than that she is perfectly healthy.
Frankly, I haven't told her yet about FNAC report, I am just telling her all these tests are important to conclude what that tumor is. I will wait till i have the options from doctors and ready to present them to her. thank you again.

IMPRESSION: Scan findings in this case reveals:
1. Minimally hypermetabolic spiculated soft tissue lesion involving right lung parenchyma, as
described – s/o primary mitotic pathology.
2. Few metabolically inactive lug nodules – likely metastatic.
3. Faintly hypermetabolic mediastinal lymph nodes – suspicious for metastasis.
4. N0 abnormal Hypermetabolism elsewhere in the body and regions under view in the present
study to suggest other site of metastases.
As compared to previous scan report (dated 06.08.21) there is no significant interval change in primary lung
lesion and lung nodules. No appearance of new hypermetabolic disease in present study.
@sahiltaneja- Oh, dear. I feel very strongly about letting patients know all the facts and then letting them make decisions with loved ones.
May I ask why she's not informed of her test results?

2 ReactionsActually it works kind of different in India :). Even the doctors are more comfortable talking to patient's loved ones instead to the patients. Because, inhere, the word Cancer is a very scary word, it makes a person more sick than he/she really is. I have told her about the tumor and possibility of it being malignant, but the final biopsy results are awaited. I wont take any decision without telling her, just want an holistic view and then can make her understand what is the disease and what are her options.

I understand. Good for you. The word cancer is extremely scary. I know as I have a long history with the word. It is now my 25th year. Yes, cancer can kill but the type of adenocarcinoma that I have is different and it is very indolent. I am lucky even though as of this post I have had 5 lung cancers. Treatments have come a long way and there is hope for a lot of people.
Please stay in touch and I would love to know the results of the biopsy. Has a pulmonologist given your mom an inhaler?
