Uterine clear-cell carcinoma, chemo treatment unknowns
Hi there! My mum has been diagnosed with Carcinoma of uterine body pT3bN1aM1 (Metastases of clear cell carcinoma in one of the ovaries, uterine tube, the greater omentum, lymph nodes). On 1st of March she had surgery: radical hysterectomy, lymph node dissection along the iliac blood vessels, omentectomy (Kyiv, Ukraine). She was discharged from the hospital and recommended chemotherapy. In different clinics in Kyiv we were offered 4 or 6 rounds of Carboplatin + Paclitaxel. Is it a right (internationally proved) schema?
One complication for choosing treatment is deep vein thrombosis (DVT) in her legs. In one clinic we were told to wait 2 weeks taking Rivaroxaban 20 mg until starting the chemo. In another clinic, we were told to start chemo next week because it’s dangerous to wait in the long term. I’m very concerned about different suggestions, considering and thinking about the long term and short term risks...

thought I replied to these but thank you all for your responses. I am all done and on the road to recovery. I am pretty exhausted and my bladder is not happy and I will have to wear stockings this summer as my ankles are swelling but hopefully that is all temporary. Hope you are all doing well. Eileen

Hi Colleen, I went back to work today! I am still exhausted but will see 2- 3 patients a day in my private PT practice. I still have some side effects I am recovering from and will try to balance getting rest! How is everyone else doing?
Jubilation, @ejohn. Sorry for my delayed response. While you were returning to work, I was taking a short summer vacation.
So...how has the return to work been going? PT can be strenuous. How are you pacing yourself?
Hello @ejohn (Eileen), I'd like to offer my congratulations on returning to work. I've been a PT patient many times over the years and I know there must be tremendous physical and emotional stamina on the part of the PT. I'm heartened to know that you are taking it slow at work and can imagine how much joy you might feel at helping others to improve their quality of life. It must be an ongoing decision for you on how to balance each and every day, one day at a time, right? Since you are in private practice, I imagine you can decide how to schedule including rest periods for yourself between appointments? Limiting your practice to one or two days a week?
Thank you Colleen and Helen for your encouragement. I was ready to get back to being me. I am still fatigued but do best in the morning. I have vacation next week and will try and stay at no more that 3 patients a day for several months to get my energy back. I am working Monday - thursday and very grateful it is working out. I was just told I have a high grade. Endometrial carcinoma. It is a grade 3. However the cancer stayed within the uterine wall. So it is a stage 1A grade 3. I hade radical surgrey and one round of chemo which I had a horriable breathing attack. So now they want to start a new one. As long as I am 1a 3 why do I have to do chemo. I want a second opinion and my doctor said to go ahead but still do chemo this week. I am confused. Please help
Hello, @lou277. Do you know the type of endometrial cancer you were diagnosed with. Is it clear cell carcinoma? Since you had trouble breathing during or after the one round of chemotherapy and they know that now is your doctor thinking they will use a different chemotherapy? Or perhaps give you medicine so that your breathing will be stable during the chemo?
We (including myself) are not medical experts here on Mayo Clinic Connect so I can't say why your doctor wants you to have chemo. I do know that Grade 3 is a high grade carcinoma like you wrote so I'm thinking that the additional treatment beyond the hysterectomy is recommended for this reason. Unless you can get a second opinion within the next week it seems like your doctor does not want you to wait on this. Still, it is always your choice of what you want to do.
Did you ask your doctor the question you are asking here? If I'm Stage 1a, Grade 3 why do I need chemo? It's a good question to ask and I hope your doctor or the cancer team will take the time to answer this question and others you have.
Won't you please come back here and tell me what your doctor says and what you decide to do?
Lou, did you get another chemo treatment this past week? Have you sought a second opinion?
