First SBRT session coming up

Posted by N227 @n227rv, Oct 13 7:08am

My first SBRT session is coming up in two days. Wish me luck. For the record here is where I am at right now..."74 y.o. man with cT1cNx unfavorable risk prostate cancer (GS 3+4 =7, 2/2 cores, iPSA 16.7, MR showing PIRADS 4 lesion right base, probable ECE; PIRADS 4 right apex, broad abutment, no ECE 48cc gland with concern for possible bone lesion in the L posterior acetabulum, PSMA PET negative for mets. IPSS 8, SHIM 14)...All five radiation treatments will be done at Memorial Sloan Kettering in New York. By the way, the bone lesion, whatever it might be, did not light up on the PSMA PET scan.

SBRT is an amazing treatment modality. With your diagnosis are they including ADT?

The hardest part of radiation therapy for me was the full bladder part, especially if they took me a few minutes late for a session. There was no other issue or discomfort at the time, though a year late, I was one of the unlucky tiny minority who developed long-term radiation cystitis and proctitis (even then, only minor annoyances at worst).

Best of luck!

I may be in a similar situation but wondering how confident doctors are using SBRT to treat possible EPE? All the best.

My brother had five sessions of SBRT at 77. The only problem he had was that it caused urinary issues, Slow flow, which Flomax every other day Resolved. I know a lot of other people that have had SBRT and Had few problems from it.

I was curious I had IMRT 34 sessions. This was to the prostate bed after a prostectomy 3 to 4 years after. I did well for a few years, minor light leakage. Then a few years later I started gradually using 4 to 5 pads a day. I only get at best 3 to 4 hours of sleep. Last night 21/2 hours. Just wondering do you have this problem or similar

@northoftheborder The hardest part for me was also the full bladder part and I may be developing long-term cystitis and proctitis as I get up every few hours thinking I need to pee or do a motion. I also have ED after three years of ADT. Still as Gleason 9 I am still alive 6 years after diagnosis, still married and able to walk, run, skip and jump.

@jrs619
This is known possible side effect of RT - worsening of incontinence or developing one.
I am so sorry that it happened to you and that it is effecting your sleep too : (.

@jeffmarc had that happen several years after RT and he resolved it with surgery, maybe you can consider dong that too.

Wishing you the best of luck 🍀

@roseat Yes! Entering my 6th year after a stage 4b diagnosis myself: still here, and able to bounce my new grandson, grow food in my garden, play classical guitar, and prepare (Canadian) Thanksgiving dinner for 7.

The side-effects of radiation, ADT, and Apalutamide fade to minor nuisances in comparison.

I had this same problem. 3 1/2 years after the prostatectomy my PSA started rising, so I had salvage radiation. 6 years later I started having incontinence problems. It got progressively worse over the following 6 years.

I started using a Wiesner penis clamp when I went out. That eliminated the need to wear a pad for up to two hours at a time.

I finally got tired of it and in June I had an AUS (artificial urinary sphincter) put in and I no longer have to use any pads at all. Test done. That device works.
