HEMOPTYSIS ....Nebulizing, air way clearance and huff coughing

I had two episodes of blood that came up today.
Not much, just a tiny amount and streaks, no clots.
Nothing feels irritated and swallowing OK. Feel O.K. also.
It came up after what I hope was a huff cough but I know I also find myself doing a hard huff cough. Not good.
Not sure if it is from the lungs or esophagus. However, I will assume it is from the lungs, for now.
I know about hemoptysis from reading posts here on Mayo.
I sent a message to my pulmonologists today.
Thank goodness I was able to read previously in threads, and today the posts about Hemoptysis and Exercise, so that I am not frightened...... just not happy.
I did not do therapy today however when I did clear what was clearly in my throat area today, in between the two incidents (Noon meal first episode and Five P.M. meal) I did not have blood.

What do the pulmonologists say about doing our therapy,....the nebulizing, air way clearance techniques and huff coughing if having or seeing blood ..... related to the lungs.?
Do we continue with therapy, and all, or give it a day or two to "heal?"

@petry That is so common, "so overwhelmed. "
I don't use albutorol but it is my understanding that it is to help open the airways before you nebulize. With things like this, especially if needing information quickly from good websites like Mayo or Cleveland Clinic or the company that makes the albutorol could help with the information, I would think.
You will probably have a reply shortly from those who use it.

I was instructed to use albuterol 15-20 minutes before nebulizing saline and airway clearance to open the airways. It also seems to thin my mucus a bit.

Hello . I wonder if someone can help me with this question is have BE/MAC. My docto at the Infectious Disease clinic put me on 3% saline solution 2x day along with the Albutorol that i am already doing , do I do saline solution right after the albutorol? I got no further instructions , I was so overwhelmed I forgot to ask him . Please could I get some ideas . Thank you

My understanding is that it is advisable to wait 10mins after taking the albuterol before nebbing to allow the medicine to open up the airways.

Hello, Thank you. Thank you! When Inget a flare, Hemoptysis is my main symptom. It’s not more than a tablespoon but it’s always scary. I was diagnosed w Bronchiectasis and Mac 10 yrs ago but was misdiagnosed when I coughed up blood years bf that.
I just now read the recommendations for Hemoptysis from NIH that you posted on June 29th, 2026. I am going to share them w my Pulmonologist and get his feed back bc he never told me to stop my airway clearance, huff cough etc. when I have a flare. In fact his triage nurse suggested I > to 3 x a day instead of pausing for awhile.

@jessed Just goes to show you that sometimes medical professionals come up with their own philosophy of how to handle things, and can differ from one professional to another and probably not with good common sense, sometimes.
If one bleeds you give it time to heal and stop sending the saline into our lungs to help to cause coughing and possible irritation to the lungs with both.
My belief is that many of our local doctors do not have the time to keep up with all the learnings that get published each day, month, year etc. and are not up to date on matters from those who do research, studies and concentration on one type of Chronic illness, such as Bronchiectasis to understand all that needs to be done, not done, followed etc. etc.
Again, glad you are reading some of the threads here.
Hope you are able to give thought to finding a Center of Excellence for you to get second opinions with all.

Barbara, thank you again for your thoughtful response. After I was dx by local pulmo, I found and we traveled to UNC chapel hill Bronchiectasis -
NTM center. Fortunately, Dr Daniels suggested Aerobika w 3% saline BID. I didn’t have another episode for about 8 years. She could no longer see me virtually so I found pulmonologist at Vanderbilt that had list of expertise w Bronchiectasis NTM. He’s been great. Vanderbilt pulmo just got certified as center of excellence. But I often ask questions, such as Dr McShane’s management, vests, and now I sent him the NIH report. I can’t help myself even tho I know TMI but you are so helpful. Best, Jessed
