How long ago did you have the radiation treatment? Maybe your PSA is just a bump which is common with radiation. Plus it does take time for PSA to drop with radiation because after all you still have a prostate producing some PSA . I think your doctor is right the % of Free PSA is of no concern now, only the total is what matters at this point.

I think the next PSA test should be lower. What did your treatment consist of, anything besides the radiation? Like hormones, brachy, etc?

One of the worse things with any type of cancer is waiting for tests & results. It's so stressful. Hang in there.

It can take up to 2 years to reach the PSA nadir after radiation. Key is the trend should continue down. Free PSA is irreverent after radiation. It often shows lower values than anything before radiation. Should just get Total PSA from now on. At a 3.0 PSA I have to assume you avoided ADT.

Your PSA is pretty high for the amount of time after your radiation. I have heard from people that took three years to get down to the minimum, That is not the usual, but it happens.

Your PSA free is quite high risk since it should be above 25%. The thing is you’ve written 7L Was that a typo. The L. doesn’t seem to belong. I assume you mean 7%.

If it were me, I would want a PSMA PET scan to find out what’s going on. I might wait till the next PSA test, but with A PSA of three you should be able to get good results from a pet test.

I think it would make sense to get a second opinion on what’s going on. The doctor does not appear to be aggressive enough with what appears to be problems. If you mention where you live, I might be able to point you to a doctor that’s in your area and highly respected.

Regarding % Free PSA —> When PSA is in the gray area - 4.0-10.0 - and MRI results are indeterminate (PIRADS 2 or 3), % Free PSA provides additional information as to the nature of the disease and whether to biopsy.

PSA comes in two forms - attached to certain blood proteins and unattached (i.e., “free”). % Free PSA is the ratio of unattached (“free”) PSA compared to total PSA in the blood. A higher percentage (above 25%) suggests benign conditions like BPH, while a lower percentage (under 15%) indicates increased risk of prostate cancer when total PSA levels are in that borderline range.

Have you already had treatments for prostate cancer? If so, what treatments?

@jim18 yes my urologist told me i didn't need it.

@jeffmarc Thanks for the feedback i live in Miami. and on the results it said 7L maybe it was a typo on there end. as far as the PSMA PET i will ask i get tested again for my PSA next month.

@brianjarvis i received 25 sessions of radiation which ended March 18,2026 it was a bit challenging between the nausea and throwing up it was driving me insane. I still get some pain i was told that was normal but dam it frustrates me.
