Good news and a question about T levels

Got my 1-year post radiation and ADT blood work yesterday and the good news is after 4 years my PSA is finally undetectable which is great news to me. 3 months after finishing tx. my T had recovered to 230. Started Tx. at 250 so a low T guy is my norm. My test result this week stated my T is now 175 and I am feeling the side effects of this. Question- Is this normal after ADT and radiation or pointing to a Pituitary Gland issue? Thinking of persuading an appointment with an Endocrinologist for further testing. Any experience with this out there? Thanks

Great that your PSA is so low!

You indicate being 1-year post-radiation with undetectable PSA. That’s unusual to be undetectable 1-year post-radiation unless:
> it was salvage radiation, or
> there is still ADT in your system.

But, since you say that your T is recovering, the ADT must be out of your system. Was your radiation primary or salvage?

It is “normal” following ADT for T levels not to return fully to where it initially was. But, since yours started out low (250 ng/dL), “normal” can be lower than that.

T recovery from ADT can depend on a number of things:
> how old are you? (Age matters)
> how long were you on ADT?
> how long have you been off of ADT?

@brianjarvis I had RP 4 years ago and never went undetectable. BCR after 2 years and treated with salvage radiation and 6 months of ADT. I used Orgovyx achieving low single digit T for 6 months. Finished July 2025. 3 months later my T recovered to 230. 250 was my starting point as I am a naturally low T guy.
I had my 1 year PSA and T test this week. My PSA is undetectable, Ultra sensitive test .014. My T has fallen to 175 which is my concern in 3 months. I will get retested.
Wondering if others have experienced their T falling after it almost being pre ADT levels a year later. This is my concern. Is it normal or do I need to see an endocrinologist for Pituitary Gland testing for other disease?
Interested in others experiences if this happened.

I’ve heard many times from people That had their testosterone go up and then down, It seems to be a pretty common problem. It usually recovers after a few months.

Great to hear you have undetectable PSA without the drugs.
