Many years since radical prostectomy: PSA values rising
I had radical prostectomy 25 years ago and controlled years of the psa values. in the last four years those values have increased from 1.1 to 5.2 in four years. Done PET test with nucleotide and found activity in the pelvic bed. The urologist+oncologist suggest ADT and IRMT for 2 years.I am 87 years old with ,as often usual, kidney problems and blood pressure medically controlled. I have read that the side effect of the ADT are often worse than those from radiation treatment.
Given my life expectation, should I simply do nothing?

Most of my mood problems are the incontinence issues. Just stressful going in places and especially now with my long walks in this heat I got to really watch it don't get too wet. Overnight I do pretty good I think, last week or two overnight's been .3 to .8 oz. Pad actually feels dry but it did gain weight overnight. Big difference during the day lots of stress incontinence. And I'm doing the kegel exercises, but doctor still won't refer me for pelvic exercises until 3 months so I got another month. Then all the anxiety has caused me to start having PVCs. But I'm trying to be positive, things could always be worse. Hopefully your procedure works well! Thanks for your advice and I always look forward to reading it. I thought about getting something called a mash or something, but they said they don't do them until a year.
Jeff, Do you think the radiation caused the incontinence problems?

I had a prostatectomy and about four years later I had radiation. I didn’t have any problems after surgery. Six years after radiation, I had incontinent starting. Could’ve been due to both could’ve been due to radiation. There’s no way to really pin it down.

I have no problem at night with leaking. Don’t have to wear anything for it. During the day, I can fill a pad in two hours If I’m running around doing stuff.
I also get PAC’s or PVC’s Every few days. Sometimes it gets real bad. I have no anxiety, however that’s just due to having taken abiraterone And it’s screwing up my heart in the past.

1 Reaction@diverjer - with respect to the pelvic floor referral, it may be worth asking that your care team give you the name of the therapy center they’re going to refer you to so you can at least get some appointments in the books now for that 12-weeks and beyond point.
I say that because pelvic floor therapists are notoriously booked out for months, and they may have a hard time fitting you in any time soon. You may walk in there at 12 weeks with your referral, and the scheduler may tell you they don’t have anything available…for another month or two.
I had the same recommendation from my care team - I had to wait 12 weeks before I’d be referred (probably an insurance thing more than anything else), but they gave me the referral at 8 weeks, and even found me a therapist in my area (I live over and hour away from the cancer center).
I called them immediately, and the soonest they could get me in was a month later, which happened to be the 12-week point, but the soonest they could do my second appointment was three weeks after that. Luckily, there was a cancellation and I got in about 10 days later for my second one, and then more or less weekly after that.

You may want to explain that you are a post-operative prostate cancer patient with subtle, well-thought-out comments on your priority as a patient to be fit into the schedule between all of the postpartum mothers. I think cancer trumps postpartum incontinence.

I do not understand why that would be the case ?
It is not "cancer" incontinence , it is "post surgery incontinence" and incontinence is incontinence. Not to mention that young mothers have million things to do and bend and squat about hundreds time a day, and carry heavy things (baby, stroller, baby carrier, groceries). They take care of a newborn baby and possibly several older children, do tons of errands, cook , clean, some even work full time AND are expected to be "sexy" for their young and "eager" husbands.
If anything postpartum mothers should have a priority .
