PSA dissapointing
My most recent PSA increased 30% over the last 3 months from 6.3 to 8.1. However, over the course of 16 months it has fluctuated from 8.1 to 4.4 and now back to 8.1 again. I've tried supplements from the evidence zone, exercise, diet modifications and reduced alcohol but nothing seems to have a significant impact on decreasing PSA . My most recent MRI was a Pirads 2 with no focal lesions and a prostate volume of 70ml. So I guess I'll continue with the PSA until my next MRI in October and then see if anything has changed. I'll be 63 soon. Right now I'm just trying to manage some anxiety but if the MRI shows any changes or suspicious areas I'll do a biopsy.

I would not delay too long. We almost waited too long (PSA every 6 months), jumped from 8 to 9. Did an MRI with dye. Mass had doubled. Took two months to get Robotic surgery. Got clean margins by 1mm. Should have had the surgery 6-12 months sooner. Get my 3 month PSA test next week, fingers-crossed!
@midwestjulyguy what was your pirads score?
@midwestjulyguy did your psa steadily increase or fluctuate over time?
@wheel1 Did they say what the benign lesion was? Prostatitis, BPH, or something like that?
@ezupcic Yes, it did fluctuate, but not a lot. The trend was a slow increase over several years. So it Was a watch and wait.
Gleason 3 + 4 disease. Decipher was lower risk at 0.20. Can't find the Pirads score, will have to find it. I have enough paperwork to fill two file cabinets! Good Luck!
Mayo PHX explained to me that the transperineal biopsy is used when the MRI detects the location of a lesion -- otherwise transrectal.
My PSA went from 4 to 7 to 10, but my 2024 and 2025 MRIs showed nothing (in the 1934 World Series, Cardinal HOF great pitcher Dizzy Dean was struck in the head by a baseball and knocked out -- next day the STL paper wrote "x-rays of Dean's head show nothing" thogh it was Dean who actually said it) -- maybe prostatitis.
Biopsy was suggested, but I wasn't sure that I needed (wanted?) one, so I asked for both the ExoDx and Lynx Dx urine tests. Mayo could order the ExoDs and I was able to have my PCP oder the LynxDx test for me. The results were 46% and 50%, so I opted for the biopsy which was done locally and with no pain or discomfort at all.
Finished 20 photon beam treatments two weeks ago. They also were a breeze with, so far, only increased gas and alittle more frequent urination.
@daveys what was your gleason score? When you say the MRI’s showed nothing is that a Pirads 1 or 2?
2024 PIRADS X
2025 PIRADS 2-Low. Clinically significant cancer is unlikely to be present. Sequelae of prostatitis. No suspicious focal prostate lesion.
