Exodx test
Recently I had an mri for an psa of 5, they found an .8 cm wedge lesion and said fi di gs consistent with prostatitis and gave it a pirads 4 rating. He wanted to do a exodx test to determine whether to proceed to a biopsy. What scores are worrisome and how accurate are they in predicting aggressive cancers. Say if you get a 30 score does that mean higher probability of higher Gleason score than 7.

I will see what happens with me over always been extremely active so
hopefully that will help my situation if it gets to that point currently

As Jeff indicated the PSE has a higher accuracy, but for the time being await the ExoDx results. You don’t mention your PSA history or from what it jumped to the 5. If you have a prostatitis infection it would have risen. If your ExoDx is low continue your PSA monitoring I would recommend quarterly depending on your history and that jump to 5 if it was a large jump from your past reading’s. If your PSA continues concerning regardless of your ExoDx score maybe get the PSE

Happy to share my experience and I will say their is a lot of experience on this board.
I started 3 years ago at 65 4.1 retest 2.2. Then 3.85 then 3.74 then this
year 5.09 then retest 4.99, mri 4 with wedge shaped lesion .8cm summary
noted findings consistent with prostatitis, psa density. 16.
Is there a decent chance this could be benign.

Certainly could be benign. My PIRADS 4 was benign, fortunately it led to my biopsy which did find my cancer in other areas. It is clear prostatitis issues in patients can reflect in the MRI.
I did read that wedge shaped lesions are indicative of prostate infection but can be malignant too, but not as much as round irregular shapes.
I guess I will see what the exods score says and let you know when I find out.
