Post Radiation
Stage 4B Metastatic Prostate cancer diagnosis in November. 9 Gleason score. Age 55. I've done Docetaxel and I'm currently on Nubeqa and Eligard and will finish 28 rounds of radiation to my prostate in about a week.
Two questions...
1. My main issue right now is extreme fatigue. I'm exercising every day and still working. What can I expect moving forward as I expect to stay on both medications indefinitely.
2. My PSA seems to have stalled around 1 (it was 6000 at diagnosis). Any experience with not reaching "untraceable" as a baseline PSA?

The chemo is probably what’s giving you most of the fatigue. Some people do get a lot of fatigue from Eligard, But chemo is really a problem. That fatigue can last even up to 10 days after getting chemo and you’ll get a few days to feel better and then go back and do another treatment.
You have a very serious case of prostate cancer. It could take a few more treatments to get the PSA down further, but it is possible it may not move. You are on the best treatment available at this time. Your PSA came down dramatically, so give it a chance. After 16 years of prostate cancer, my PSA has finally stayed undetectable for 31 months. With my genetic problem, it keeps coming back, And has reoccurred four times. With your case of prostate cancer, you’re doing really well.
Have you had hereditary genetic test testing? You got prostate cancer quite young, That can be due to genetics. There are treatments for some genetic problems. Do you have cancer in your family? Breast cancer, pancreatic cancer and prostate cancer can all be caused by genetic issues. I got it at 62 because I have BRCA2. My brother got it at 77 because he doesn’t have it. Our father died of it, increasing the chance we would get it better than 100%.

1 ReactionStage IVB, Gleason 9 here too, although my PSA at its peak was in the 30's. ADT/ARSI since June 2025, radiation completed October 2025. Lowest my PSA went was 0.19 before starting to rise again. Never reached undetectable. Haven't had chemo yet. But...I started out at 5970 points lower than yours so you might just need a little more time. Best wishes.

6 ReactionsAwhile ago I saw where Mayo recommended taking Wisconsin American Ginseng for fatigue. I ve been on it for 3 years. Still need an afternoon nap. Give it a try BID.

I was told it helps with brain fog, nobody ever mentioned fatigue. Ginseng, however, does have an effect on fatigue for many people.
I’ve been using it for quite a while, Going to see if fatigue is of something it actually does, I never noticed it. I’ve been using it for at least six months. Have you noticed a reduction in fatigue due to it?.

1 ReactionHi Jeff, I take one pill in the am-500mg. I get tired by 1-130pm. I m sorry, can’t answer your question cuz the bottle says: take 2 capsules with water 2 times daily: which I don’t do. I drink a cup of ASSAM tea after the nap as it’s loaded with caffeine to finish off the day. Maybe I should follow the bottles direction to see if it makes a difference. That’s the best answer I can give ya.
