Tadalafil Benefits?

No. I have INC and ED. 6 years since surgery. No other treatments. I waited 5 years to work on ED issues. Tadalafil was given at 5mg daily dose with 20mg for quicker response. Tadalafil daily dose has given me back some Nocturnal erections but no help on demand. I have had in the last year no change to my INC.

2 ReactionsBeen on 5mg since early Dec of 2025. Don't know if it's helped urinary function/mild incontinence. Doesn't do much for ED caused by Lyrica. Cheapest source is Mark Cuban's Cost Plus drugs.

4 ReactionsI was on it for a while. The RO thought it would help with LUTS. I had mistakenly thought he was prescribing it prophylactically to limit penis atrophy. I developed weird muscle and joint intermittent pains that would sometimes affect my hip, sometimes my knee, sometimes my thigh muscles, other joints and muscles, etc. I read a report on this forum of tadalafil being implicated in a patient reporting very similar symptoms so I stopped taking tadalafil. His symptoms stopped immediately upon stopping the drug. Mine persisted, but very slowly have almost disappeared. I have no idea if stopping tadalafil in my case affected the pains I was experiencing.
I didn't notice much of a difference with my urinary symptoms when I stopped the tadalafil. I will be experimenting with taking it again to see if my pains start up, but I'm not in a hurry...

5 ReactionsNo surgery. I had urinary issues during and after radiation, difficulty starting even with a strong urge and needing to go frequently. I had issues with Flomax and had to quit. Tadalafil had 2 potential benefits and was reasonably inexpensive, so i asked my RO to prescribe 5mg. I was happy with the results as to difficulty it didn't do much for frequency

2 ReactionsI'm 2 1/2 weeks post surgery and in-process with post-catheter stress incontinence, which is super challenging. But, I guess this is to be expected.
I don't know if Tadalifil helps with urinary issues, but I will say it's a game changer concerning ED. I started Tadalifil about three weeks prior to surgery so that I could figure out what side effects I would have. Everyone is different. For me, I had severe muscle ache in the thighs and lower back. I figured out that if I took it in the morning that I didn't get as many aches and then that pain went away within a few days. So, my body adapted. The benefit of Tadlifil is that it stays in the body longer than Viagra.
Post surgery, I started taking Tadalifil once the catheter came out. The concept that I have seen on lots of medical sites on post-RP surgery penile therapy is the goal of the drug is to encourage blood flow in the spongy tissues of the penis so that you don't develop fibrosis (scar tissue) in those tissues, which would lead to long term ED. Meaning, the normal mechanism of nocturnal erections to keep those tissues healthy is over for a good long time during recovery. Tadalifil helps mitigate that somewhat.

I will report that I had some response to Tadalifil post surgery (even the very next day after the catheter was removed. Not full-on erections by a long shot....but enough life downstairs that I have hope as the nerves recover over time, that there may be hope. We'll see over time.

2 ReactionsI've been using Cialis for about one year, taking 5 mg per day, except for taking 20 mg twice per week. I have stopped taking it two or three times to test whether it is making a difference in ED, and I see a definite difference, so I have continued taking it. I did not know it had an impact on incontinence. 16 months post surgery, I am still having trouble with considerable leakage when I am standing or otherwise active (not sitting). Is there a connection between Cialis and incontinence?

2 Reactions@ramblinman Not incontinence but urethra restriction. Besides, the marketed ED benefits a Cialis side effect is relaxation of smooth muscle that improves urine flow. I was able to drop Flowmax with daily Cialis, but that will not occur in severe cases (weaker effect). The primary reason it is used post RP is to increase blood flow as mentioned.

3 ReactionsI started on 5mg every other day, then after about 6 weeks, my physician increased the dose to 22mg. I have been taking that every other day for a year with absolutely ZERO results, despite my physician claiming that he "preserved the neurovascular bundles" necessary for erections and sexual activity. How do any of use know "for sure" that any of our urologists/surgeons "truly/actually" did preserve our neurovascular bundles? They put it in their surgical report, but there is no way to check...there is no follow-up test to verify it. It's all so utterly frustrating and defeating.

I wonder if these reports start as pre-filled templates that the provider “edits” and sometimes fails to make corrections.
My prostate path report stated that the cancer was confined to the kidney!! It was revised a week later. I had an MRI last year that stated my gallbladder was within normal limits. My gallbladder was removed 15 yrs ago!

2 Reactions@rlpostrp someone posted a nerve sparring Dr video last week. I watched that video. His explanation of whats involved with nerve sparring removal. Almost like pulling a piece of gum out of a full head of hair without cutting or breaking one piece of hair plus getting all the gum out.
