Has anyone had experience with duloxetine and gabapentin? Duloxetine and Tramadol?

My doctor prescribed Gabapentin (300mg 3x a day) for my IPN approx. 5 years ago. The burning and stabbing pain lessened somewhat, and I learned to live with the round the clock pain. Two years ago my doctor added Duloxetine (30 mg. 2x a day) as well.

During the day, and into the evening I manage. Does the combination work? I’d have to say yes from my experience. Anytime I run low on one of the medications, and have to skip doses or ration what I have left, I notice an immediate increase in severity of pain.

Let’s all promote an awareness of peripheral neuropathy, hope for a cure, and an increase in proven treatments.

What about the burning that comes with idiopathic neuopathy? Going to ask my dr. today as I am at my wits end.

I was taking Gabapentin for my non-diabetic peripheral neuropathy, at one time up to 1200 mg daily (after knee replacement) and then tapered down to 700 daily. In January 2026 my neurologist suggested I try duloxetine 30 mg in the morning and possibly reduce the gabapentin even more. I tried it and to my surprise, it not only eased the neuropathy discomfort but reduced the intense itching I was having in my face and head and joints as well. I had tried all sorts of skin treatments prescribed by dermatologists and never connected that it might be caused by nerve damage. I managed to taper down the gabapentin to 200 mg taken two hours before bedtime. I tried tapering down to zero but spent several sleepless nights trying to tolerate the hot vice feeling in my feet and legs. The burning and itching in my skin remains better but flares up if I do heavy lifting or straining or get too hot. I'm learning to live with some lifestyle modifications such as cool showers and avoiding heavy lifting. I am nearly 79 years old and have had neuropathy for 15 years. I have Hashimotos tyroiditis and mild cardiovascular disease, osteoarthritis and osteoporosis. I exercise by walking, aqua classes and Silver Sneakers, gardening and housecleaning.

I take gabapenting 3 times a day. I fought taking this drug on and off for over a year because it is so sedative and creates mental confusion for me. I had to go back to it because it does give relief for neuropathy in both legs. I have had this condition for 2 years. What I have learned is the Vagus Nerve plays a role in my condition. When my gut is happy my leg pain is fifty percent less. I can exercise my leg muscles with almost no pain. I have dealt with IBS for years. I started really working on gut health and understanding how to reset vagus nerve. I have osteoporosis especially in my right hip. Eventually I am going to have to see a specialist about the hip. I am 68.

I take Pregabalin which works very well for me. 25mg dose each.. i take as needed up to a total of 150mg, some days i dont even need one!! Must try the THC gummies as they are legal here in Canada.

Many patients and clinicians have experience using Duloxetine with Gabapentin, which is a commonly prescribed combination for neuropathic pain that can provide better relief than either medication alone but often comes with increased side effects like drowsiness, dizziness, and brain fog that usually require slow dose titration and adjustment, whereas combining duloxetine with Tramadol is used more cautiously because both affect serotonin levels and can increase the risk of Serotonin Syndrome, a potentially serious reaction characterized by agitation, sweating, rapid heart rate, and confusion, so while some people do use that combination under medical supervision, it generally requires closer monitoring and a careful risk benefit discussion with a doctor.

I take 60 mg Duloxetine with 200 mg pregabalin daily for years. I am satisfied with the treatment as it relieves the pain and tingling in my hands and feet. I am not aware of any side effects from these two medications. I have plenty of other medications for my heart that cause diabetes and other side effects like dizziness.

@billh I take gabapenin 300 3 time day. It's great helps pain calm me really do not no the other drug I have no side effects

I continue to feel burning. At first I thought the medication didn’t work because I still felt burning and stabbing pains.

When I ran out of Gabapentin once or twice, the pain increased to much worse intensities.

How much Gabapentin did you take? I did try Lyrica and it was awful - very low dose but made me so tired I could hardly walk to the car so decided to forget it. Now, am going to dr. tomorrow and see if he could do more blood tests to see if I am lacking some more Bs. My B-12 was normal. I do not have diabetes but surely he could find something. I have been on amitriptyline for years and take it at 7 pm always to make me sleepy and dr said amitriptyline does help this and I know it is not the first med choice but by 9:30 pm my symptoms are letting up but can't take it in the day - will see what he says. I hate the burny feeling but am willing to learn how to cope without and had 3 treatments of acupuncture and called her today to cancel my appt until I see the dr. She said I need 10 treatments so we shall see as it's $50 per treatment. Anyway, I will ask my dr if he could check on her etc but she has had 15 years of experience and has a 5 star review. So, don't know what to do but ask my primary care dr questions tomorrow. Thanks for responding to me.
