Reclast, Actonel or Evista?
Went to my first of two different Endocrinologists appointments today. A nice enough Dr but didn’t seem interested in what I had to say, made comment about how he was the Dr. I understand that, but told him how my Gynecologist kind of blew me off. I told him that the Dr gave me diagnosis while I was driving my car and I had made comment about how I had been crying all weekend prior when I seen results in patient portal and that the Dr’s was response was “oh” then gave me choice of Prolia or Evista. The Endo Dr said, this isn’t a life threatening diagnosis it’s a lifestyle change diagnosis. I said ok, but as I started crying again I said, it’s not right to dismiss me. I mentioned magnesium and he said it’s a hype, I mentioned how calcium is only absorbed at 500-600mg at a time. He said that’s not necessarily true. Said only need to take 2 servings of calcium a day, glass of milk and cup of yogurt. In my head I was like what?? Mentioned K2 and he said, no it’s waste of money only the Japanese treat with that no one else. Told him I can’t tolerate Fosomax tried it 2 years ago when I had osteopenia and had severe lower back pain, couldn’t even roll over in bed. He said, but the pain went away right? Yeah after a couple of days. So he gave me choice of Reclast, Actonel or Evista. Told me to get some blood work, gave me a website to look at and look at which drug I’d like to try. He doesn’t like Prolia and said I’m not a high fracture risk so he doesn’t recommend, Evenity, Forteo or Tylmos. He said he was switching a lot of his patients to Reclast. I have next appointment with different Endocrinologist the 21st this month. Hoping to have a better experience.

1 ReactionJust wanted to give an update on my 2nd opinion I received today. Well technically 3rd if you count the Gynecologist who diagnosed me. First Endocrinologist basically dismissed me as you can read in my original post. This Dr was very understanding, listened to me and answered all my questions even before I asked them. She mentioned how she prefers anabolic meds before antiresorptive as you typically have better results. She also prefers Forteo, Tymlos and Evenity, but she said insurance won’t always allow those until you have a fracture. Which is ridiculous. She wants blood work done first to see why I have osteoporosis, wants to check my bone markers. Also wants to do a 24 urine collection to see how much calcium excreting. I felt like she has her ducks in a row. If insurance allows it she’d like me to go on Evenity. I mentioned Evista she said I could do that too but wants to see what bloodwork says first. So I feel like I’m in good hands.

3 Reactions@rudysmom Sounds like a very good doctor!! you are lucky to have a choice in endocrinologists. It can be very hard to get in to see one here. I have used both Forteo and Evista after 13 compression fractures and feel very lucky to have no further fractures since 2016

Fracture free for 10 years is good news. 13 fractures leading up to it not so much. Were you on any meds when the fractures happened? I’d like to try Evista but the blood clot part scares me. To be honest it all scares me but so do fractures. So at the moment I’m living in a high state of anxiety. Talking with a friend who doesn’t have OP but has other issues said she would just take a med and not even question it. She currently takes 5 meds for other issues. I told her I think my issue is that I don’t take any meds for anything. I’m 60 and apparently have lucked out until now. So I’m afraid of all of it, where she’s like, eh what’s another med. LOL
@rudysmom, i like your new doctor; ruling out secondary causes is a critical first step before diving into treatment. If both doctor and insurance are on board, anabolic first is the preferred strategy among specialists to "build first, then maintain". You've won half the battle already!

One advantage of HRT or raloxifene is that their effects are reversible - unlike some of the bisphosphonates - though they carry their own risks. Also, estrogen is FDA-approved for prevention rather than treatment, some doctors are more cautious with it in complex cases or for patients who've already had fractures. But i think it's worth discussing if it is feasible in your case.

I read your story. What great results. I think we need a thread on positive success stories like this. I wanted to ask Dr about HRT but she was giving me so much information I totally forgot! Remembered about it as I was checking out 🤦🏻♀️
Are you still currently on HRT? If so how long is the plan to stay on it?
@rudysmom i'm in my 2nd year of HRT with DXA coming up in a few months - will update on my progress.
Before starting HRT, i was thoroughly evaluated by 2 cardiologists and given the green light. According to a large French observational study of over 50,000 women, no significant increased risk of breast cancer was found with transdermal estradiol and oral micronized progesterone for up to approximately 5yrs of use - so that's my current horizon, will re-evaluate afterwards. If my bmd shows concerning decline, retreatment with Forteo is on the table.
