@gram13 welcome to Mayo Clinic Connect. I am terrible with acronyms. Iga is somewhat familiar to me in terms of my own Chronic Kidney Disease (CKD). Mgus sounds like an acronym referring to a related condition. I believe other Mayo Clinic Connect members have this. Would you be willing to tell a bit more about your condition so that members who respond have a better idea what you are asking pertaining to “elevated Iga”? Also I’m not sure about the “Kappa/lambda ratio is 7.42”. This sounds like lab results. Could you elaborate?

1 ReactionSo...I tried to send a reply. It said it cannot send it. I am not sure why.
@gram13
I am now waiting for bone marrow biopsy.
@gram13, I moved your question to the Blood Cancers & Disorders support group here: https://connect.mayoclinic.org/group/blood-cancers-disorders/
In this group, you will meet many members who live with MGUS, like @pmm @kayabbott @projfan @positivethinking @dondi2101 and others.
Gram, when will you have the bone marrow biopsy?
@colleenyoung
Friday
I have IgA Kappa free light chain smouldering multiple myeloma (SMM) (my MGUS [monoclonal gammopathy of unknown significance] progressed to precancerous SMM last summer after 10 years of MGUS). MGUS doesn't progress to SMM or MM for most people. IgA is a type of immunoglobulin, elevated means it is higher due to inflammation, your bodie's response to illness, MGUS, or other. My current kappa/lambda ratio is 75. Bone marrow biopsies can be a bit scary (no pain with mine) but give a lot of information about the cellular composition of your bone marrow, and potential genetic risk factors.
Mine ratio is 74.
@kayabbott
My ratio is 74. Waiting for blood work results from today. Looking at platelets particularly.

1 Reaction@gram13 It is hard, the waiting. The doctors look for different thresholds, CRAB symptoms, M spike, genetics, and others to see if/when SMM might turn into MM. In a way I'm lucky, I will get treatment before it becomes MM; most people have MM detected after CRAB or other symptoms. I'm 72 and my kappa level is 932 mg/L (378 6 mo before), and still not at the level for treatment.
