Premarin once a week

Posted by grrranny @grrranny, Dec 25, 2025

I'm 82, and was using Premarin vaginal cream successfully once a week for around 25 years to treat acute vaginal atrophy. But stopped (not thinking) for 6 months (busy with outpatient re-hab) when had a stroke last May. Re-started a month ago, with just one application, then forgot about it again. Last night noticed a few bright red drops of vaginal bleeding into the toilet bowl. Immediately inserted another Premarin cream application vaginally, and have had no more bleeding. Do you think I need to be examined by my OB/GYN, or just continue my weekly applications without fail?

@grrranny. We aren't medical professionals here at Mayo Clinic Connect. We do provide one another with support and share our own life experiences. I've learned that any time you notice vaginal bleeding after menopause you should see your gynecologist. While the vaginal bleeding could be related to the lack of Premarin your physician should rule out any other causes. I was 67 years old and post-menopausal when I noticed vaginal bleeding. Just a few drops but it got my attention. I made an appointment with my gynecologist. Tests later confirmed it was endometrial cancer (endometrioid adenocarcinoma). In retrospect I am satisfied that I did not wait to see my gynecologist.

So, while your vaginal bleeding may be related to the lack of Premarin why take any chances? Will you make an appointment with your gynecologist and insist on getting in soon?

@naturegirl5
The earliest I could get an app't was in a week and a half.
Good to know that you have recovered from your endometrial cancer. Did they think it was related to using Premarin or Estrace? Did you resume using it after you recovered??

@grrranny I don't know that my endometrial cancer was related to Estrace. My gynecologist said "no". It was recommended by my surgeon (gyn/oncologist) and radiation oncologist that I do not resume using the Estrace as my endometrial cancer was estrogen positive. So I have not used any estrogen products since 2019 when I was diagnosed.
