Naltrexone

Posted by diverdown1 @diverdown1, Aug 7, 2025

I discussed L low dose Naltrexone with my doctor. The research is showing that doses of 4.5 mg is having some positive results. I tried 4.5-6 mg in 2023. Although it seemed to help some, I stopped taking it due to the price as I have no insurance and Naltrexone is not available in such a small dose unless compounded at a special pharmacy. We discussed my trying it again, as I am desperate to feel better and function better. I started taking 25 mg of Naltrexone about a week or so ago. I took it at night. This was a mistake as I could not sleep so I changed to taking it in the morning. It does help. Just fyi...it can't hurt to try it for all of you who I know are struggling with this debilitating virus that has wrecked our bodies.

No, I am taking 25 mg. This is not considered a low dose. I tried the low dose 4.5 mg but it has to be compounded as the only dose available without compounding is 50 mg. So I break the 50 mg in half. This is a dose used to treat OUD, you are correct. We are just trying it to see if it helps and it seems to help.

@diverdown1 I use a whole 50 mg tablet at first I had side effects from it so I broke it in half but now I tolerate the full tablet tablet and it helps me severely reduces brain fog. A little less pain a little more easy going psychologically if you overdo it that can undo the benefits. I'm thinking of going slightly higher on the milligrams of my doctor agrees to see if it helps more.

@ladyrocc007 I agree that it helps me feel better. I am still on 25 mg. I tried 50 mg and sometimes, when I feel really awful, I will take a 50 mg. tablet. I find that Naltrexone causes similar side effects as the pills I was addicted to before I got sober (GI wise) but I do notice that I need more dopamine and Naltrexone helps with that. Let me know how you fair on a higher dose. Thank you for the reply!

Oh wow, I am on 8 mg and thought we had to stop at 10mg, but you're taking 25mg and 50 mg... Do you have links to research for that ? I'll google it for myself as well, but it's hard to see the forest for the trees !

I am doing an oxygen enrichment Trial at Toronto Western right now, which is also said to help brain fog. I have only had 3 sessions but I do feel more alert afterwards !

It's two sessions every day of 30 minutes. FIngers crossed.

For @diverdown1 and anyone concerned about the cost of low dose Naltrexone, you can save lots of money by taking it as an oral solution - it costs me with insurance coverage about ten dollars a year! Instead of more than $1500 a year with the compounding pharmacy which insurance would not cover.

You have to make it up yourself but that is easy. I take one 50mg tablet and mix with 50 ml water (using a graduated cylinder) in a small jar and let it sit overnight. That gives me a 1ml = 1mg solution, so easy to take whatever your dose is. Mine is 4mg so I use a 4 ml syringe to pull up that much liquid and drink it. I used to be on 3.5 mg a day so would use a 3ml syringe plus a 1 ml syringe and mark off the 0.5 ml mark.

@fse1973ap People taking 25-50 mg of naltrexone are taking it for addiction usually. LDN is taken for long COVID, me/cfs and similar diseases...and not for addiction.

You can get it compounded at a 24 hour Walgreens and I only pd $15. I am in WI. I just called around and asked pharmacies if they compounded medications and what the cost would be. I just started 1mg Naltrexone. They used GoodRX to get me the discounted price. Call around and see what you can do. Good luck!!

Thanks a lot for info. What do you mean “ compounded” ? Is it a couple of drugs combined together. .?

Naltrexone can only be purchased at a pharmacy at the dose used for people who use it for opioid addiction which is 25 or 50 milligrams. The low dose (.5 to 6 milligrams) is not available for purchase so must be compounded (e.g., created at a very low dose) by a compounding pharmacy. My compounding pharmacy originally made me 1 mg pills which, for me, were not that effective and were hard to titrate up slowly...so I asked for 1 mg per milligram liquid and they made it for me. The liquid has little to no flavor and at even my highest dose of 4.5 mg... it is a small amount of liquid.
