Side effects from withdrawal from Keppra

I had craniotomy and have been seizure free for almost 5 years. I have been tapering my pills very gradually with my LMD. Everything has been stable until I reached 500mg Keppra in the morning and 500mg Keppra at night. Keep in mind i do take vimpat 300mg two times a day along with keppra. The only medication we’ve been tapering is Keppra. Two days ago I experienced something. I experienced memory fog and feeling like I'm in a dream. I have asked questions i usually don't ask. But during all of that time i was alert and aware. I was able to communicate and answer questions. No one seemed to notice that something is wrong. But i do not recall the conversations or anything, its like i was sleep walking. Thing is i was alert and aware they said.Someone said i was walking in circles. I just want to make sure if this is
normal during tapering or not. I dont remember how i got to work either.

It sounds like a focal type seizure to me.
I once had a focal aware seizure, where I had an intense déjà vu episode, which seemed like a dream like state.
Hopefully, you have notified your Neurologist of this episode.
I hope you keep us informed as to where he believes happened.
I would discontinue the taper.
Did your doctor recommend the Keppra taper.

I contacted my mayo clinic neurologist and he said i should go back to my previous dose. I have a video visit with him soon, I still don't know what happened, it’s probably a seizure but how could a seizure last for hrs? I don’t mind stopping the tapering if it means i wont go through that again and keep me seizure free. I’ll let you know what he says when i have this talk with him.

Yes, it sounds like some sort of seizure activity to me, too. You didn’t say, but did it resemble any previous seizures? If it resembled things you previously knew were seizures, then it’s very likely a seizure, in my opinion. Anyway, I would stop the Keppra taper and talk to my neurologist promptly.

No not at all its a new experience to me. It probably is a seizure, my neurologist told me to go back to the dose. I agree with stopping the tapering it doesn’t make me feel safe at all.

I'm glad to hear you've already contacted your neurologist and that he/she recommended going back to your previous dose of Keppra.
To me, this also sounds like you had a seizure—a focal one. Most of my seizures are focal, and what you described is very similar to what I experience during mine. When I was a teenager, before I was diagnosed with epilepsy, I remember telling my mother that "the dream was starting again." Now I know those were focal aware seizures.

What types of seizures have you typically experienced before this one? If I were you, I'd ask your doctor what kind of seizure this might have been.

Before the surgery i used to experience a different kind of seizure which was like a deja vu sensation and i used to stare blankly and people would notice. I remember hearing different sounds like a phone ringing. But i never experienced what i was telling you about earlier. I used to come back in like 30 seconds. I was never aware and i could hear you but not able to reply or communicate. Thats why I am confused cause i never felt or experienced something like this before. I remember it being called a focal seizure.

So much of our symptoms depend on what part of the brain is affected and how it’s affected. I wish I could help you figure this out more, but they know what piece of my brain is misbehaving ( and generally why) and so I haven’t had the extensive diagnostic work up that sometimes is called for.

Hopefully your neurologist will have more good ideas. It sounds as if the Keppra was doing its job. Best wishes going forward.

Thank you for sharing more details about your previous seizures.
What I've experienced personally is that seizure presentation can change in various ways throughout a person's life—including the symptoms themselves, their intensity, and how long they last.
Initially, my focal aware seizures were very mild. I'd have clear déjà vu experiences, unusual smells and tastes, and would recover quickly. Over the years, though, things have changed significantly. The smell and taste symptoms disappeared entirely, and the nature of my déjà vu experiences shifted. My recovery time has also become much longer than it used to be.
Is it possible that something similar has happened with yours—that your seizures are presenting differently as you age or as your condition evolves? I'd definitely encourage you to ask your doctor what type of seizure this recent episode might have been.
