What to think when only free light chain ratio is abnormally high?
I went to a hemotologist because I had a blood clot and needed to know if I needed to take a blood thinner forever. I am a 68 year old female and when blood work came back, it said my free light chain ratio was 101.55, kappa light chain was 89.36 and the spep test said "a small amount of free kappa monoclonal protein (too small to quantitate) is present." I had never heard of these tests and was surprised something was listed as abnormal. I have no symptoms and all other blood work was normal. I am scheduled for a bone marrow biopsy. When only a ratio if high, and nothing else is abnormal, is this something to be concerned about or is it a monitor and watch thing?

@barbarian1 “requesting another test from diff provider like Quest” is a good idea.
Ask your doctor if it could also be a sign of kidney disease, chronic inflammation, or an immune system disorder, as this may require additional blood tests.

2 Reactions@georgiagurl how are you doing, I was thinking about you today. I was wondering, have you had your BMB yet?
@auntieoakley Thank you for asking, that is very kind. I had the BMB on this past Friday and so I now await the results of the testing. The BMB was under sedation and I am so glad for that. But, I have to say, it really did hurt to walk the rest of that day (although I did mostly rest that day). The next day it was not so bad to walk and a couple Tylenol helped greatly. Of course, the waiting time for the results is nerve-wracking but can't do anything about that. I will update once I receive. Thank you again for inquiring.

2 ReactionsI received some of my test results and they indicate 8%-10% plasma cells in bone marrow so the pathologist said I likely have smoldering myeloma. As noted above, I do have a very high kappa/lambda ratio of 101.55. I am now waiting for the FISH results to determine the risk level. I am now also scheduled for a PET scan to determine if there are any lesions. I initially went to the hemotologist solely to determine if I had Factor V (increased likelihood of getting blood clots) and now I may have smoldering myeloma? Geez. What a ride.
