Optimal time to nebulize?
Does anyone know when the optimal time to nebulize? Or is there any? I usually nebulize every morning about an hour after getting up. My lungs feel full, an I realize that the mucus has settled due to the lying position all night. I wait the hour because I must have that cup of coffee first. Then Levabuterol wait 10-15 then 7% and the mucus does come up good. My main concern is in evening. Do people neb before dinner, after dinner. Just before they go to bed? I usually eat dinner 4-5pm then I wait until about 6:30-7 then start nebbing which takes 45-60 mins then bed. Again after nebbing in evening my lungs usually feel pretty good and I get a good amount of mucus out. Then bed. When I get up next morning lungs feel full again. Anyone else have this issue? Are the times I nebulize an issue.? Thank you one and all.

It's really up to you what works. Most agree that morning first thing feels necessary. The only part of your routine I wonder about is drinking coffee before clearance. I would hesitate to do that as to not tempt reflux. Rule of thumb is limit fluids/food for 2 hours before clearing especially if PD is involved. I usually clear pretty much about an hour before sleep. What's optimal is twice a day and you are doing that! Kudos!
Are you on Brinsupri? I'm at week 3 and it already made a difference in reducing clearance time.

4 ReactionsYes Scoop, I’m on Brinsupri and have been since 9/5. I’ll be honest I’ve often thought about coffee after morning clearance it does make perfect sense. Although I must admit I do drink quite a bit of water throughout the night, maybe 10-12 oz. Can u tell me what PD means? I should know but drawing a blank. I will try nebbing an hour or so before bed sometime soon to see if that helps also. As for Brinsupri, I would say I think I notice a difference, not in the time of AC, but in the amount of mucus & ease of expelling. Though only a slight change. But I’ll take any improvement I can get, as I think we all would. I still believe one of the most important factors in how well you do in your AC is the location of the worst of your BE. Mine as I’ve said is both lower lobes posterior. As always I truly thank you for the tips and advice, we are all so fortunate to have people like you, Sue & each other to help us manage this difficult disease.

1 Reaction@scoop great point about the coffee prior to the clearance. My schedule is very much like @spider109 - need my morning coffee (even if it is half caff with lots of steamed milk!). Maybe I need to move things around a bit and do AC first then do coffee as reflux has been an ongoing issue for me. Thanks for bringing that up.

I will be talking to my pulmonologist the end of the month about starting the Brinsupri and hope getting it etc. etc. is seamless.
Another individual on Brinsupri said they had less of a need to clear the mucus, feeling of lump in the throat etc., in between therapy sessions of nebulizing. How I hope that will be the same for me when I start the Brinsupri.

3 ReactionsJust the other day, my pulmonologist told me to nebulize either in the morning or at night.
I am using Amikacin in a vial that I draw up and add The NaCl to the syringe and then ‘shoot’ it into the nebulizer cup. Tricare for Life denied Arikayce.
It makes me cough when starting but at least I am not hoarse or coughing up blood. Dr McShane prescribed this after my first conversion of Abscessus maissilience in 2023. I was also on clofazamine and zith.
I would recommend everyone get tested to see if they are ERM 41 positive or negative. It makes a difference if you are ERM41 positive. The ‘mycin’s ‘ don’t ’work,’ Dr McShane tested for ERM 41. Boom I was positive.

3 ReactionsHi @spider109. My schedule is very similar to yours, including that first hour in the morning sipping coffee and reading the newspaper; between those two things, my blood pressure rises high enough to get started on the day 🙂
At night, I nebulize at 8:00 p.m., after Jeopardy (hey, we all have our priorities). We used to have dinner around 7:00-7:30 but have moved that back to 6:00-ish because of all the advice here to avoid reflux, but it still feels so early. When the doctor told me to nebulize twice a day, I just assumed he meant 12 hours apart, give or take.
I hope you and @scoop and all those taking the Brinsupri have positive results. It's next on my list of things to discuss with the doctor!

1 ReactionMy times seem to be however it works into my day. I prefer a routine but that doesn't always happen. When I first get up in the morning (anywhere from 5-6:30am) I usually have a cup of hot tea. Gave up coffee over a year ago. No soda either. Depending on my day and how hungry I already am, I may have a piece of toast and avacado. Then wait a couple hours to nebulize. However the earlier the better for me so I can get on with my day.
I always stretch and sometimes other exercises and/or weights 3-5 lb hand weights while nebulizing in the morning. I find it gets things moving both in my lungs and my body and really passes the time. With deeper breathing since exercising too my nevving usually takes 20-25 min. Then with soaking and cleaning, sanitizing in instant pot, (cleaning the pot and lid too) waiting for the instant pot to do its thing and then moving to baby bottle sanitizer it's about an hour and half to be able to totally walk away. Of course while things are soaking or IP running I am doing other household things or reading. But still ... toes to the house until it's all done.
In the afternoon or evening it's really all over the place depending on the day. I like trying to start the process around 4:00-4:30 so while things are cleaning and IP running I can fix supper and be able to eat around 6 when hubby gets home from work. Then have the evening to relax. But sometimes I don't get the afternoon session done early enough and too hungry to wait - so it may be 7:30-8:00 or after and that sucks for me! But then im ready to do NOTHING.
Im retired. I cant imagine working or having children at home
And having to do this everyday for the rest of my life. UGH
I'm blessed not to have Mac or any infections and I hope to keep it that way ... God willing!
