Has anyone heard of RLMS (right lobe middle syndrome)
August 2024, my PCP requested a
CT scan and findings were multiple pulmonary nodules, which then it was recommended to do another CT scan in April 2025. January 2025 I was diagnosed with Pneumonia, one month later my symptoms were similar, other than that awful tasting phlegm/mucus, and I’m struggling to breathe, and my lips turn a dark bluish color. X-ray revealed possible COPD. Doctor’s notes say, “The imaging shows right middle lobe collapses.” It’s painful and scary. I was prescribed, Prednisone, 5 days worth, @ 2/1 times daily. Albuterol Inhaler and Doxycycline, 7 days worth. I’ve completed both meds and it’s still painful and I’m still struggling to breathe. Anyone have any advice?

4 ReactionsIt's terrible that you have had to wait so long to see the pulmonologist. I'm so sorry! I had s wonderful one when I ended up hospitalized for triple pneumonia. So I just kept seeing him. I hope the one you get is worth the wait.

I hope you can get in soon. This might be rude, if you are able I would consider going to the office and give your insurance info and a little information on what you have going on. They will ask if you want to set up an appointment, tell them yes for today and go over and sit down.
I'm not trying to scare you. But I waited to long it was my own fault. I had my upper two lobes out of my right lung. They left a little bit at bottom, but it's junk. I wish you all the best. Jeff
I see you’ve set up an appointment with the pulmonologist which is good. When I’m in need of more urgent care from my pulmonologist I contact him directly through the patient portal with a brief summary of my symptoms. Go to their web site and connect with the portal. Check with the office if there’s a specific procedure or password needed. It’s always expedited care for me. Hope you feel better soon!

1 ReactionYes, had Bronchoscopy with lavage this past week and Dr. told me that is the issue. (When i fist saw him and he read my scan, he said, that it was Avium and not to worry about it, I told him i have been coughing for months!) Avium is bacterial. He took cultures and those will be back in 2 wks time. Meanwhile I have been testing positive with sputum tests for candida. I have had two lung ct scans in the past 3 months. I have been coughing, continually-unable to sleep and he and my primary keep treating my symptoms. I have been frustrated with this pulmonologist as I feel he has been dragging his feet: waiting for sputum tests to come back then telling me I haven't done them correctly because all he was seeing was candida in the flora. Meanwhile, i had a second lung scan. Lung scan did indicate right nodules in upper and middle lobe suggestive of small airway disease. I felt he was blaming me for not doing test properly. He finally was satisfied with 3rd and 4th sputum tests to go to Bronchoscopy with lavage. He said he could not get the tube past middle lobe, it is so conscrited. He is not a great communicator. Now my question is what has caused this RMLS and if it is in fact, candida...or candidiasis...will treating/curing this solve the issue of RMLS? While i was in recovery room, he mentioned that he might want me to see Thoracic surgeon. No more mention of Avium. I'll get back to this thread when i have culture results.
