Has anyone experienced incontinence after HOLEP surgery?

I recently had HOLEP surgery at the end of 2024. One of my main issues nine weeks later is incontinence or leakage. Been doing kegels but it still persists. Has anyone else experienced this and for how long?

Any updates on your experience with Holep? I am six months past surgery and still have leakage when on my feet. The doctor seems troubled and is not very helpful at this point. I did about six weeks of P.T. , but not much changed. Considering looking into a urology specialty hospital for some guidance or input. My doctors lack of answers concerns me. Basically dry at night then leak all day, more so with the amount of fluid I drink. Any help out there?

I am 8.5 months past my HOLEP. Physical therapy has been greatly helpful but you need to do about a half a dozen exercises plus kegels to see benefits. I'm down to one pad a day. Time and continuous therapy will pay dividends.

Thanks for the reply. Can you be more specific about your half a dozen exercises you do. Do you mean doing the kegel during an exercise or separate exercises in addition to your kegels. I have been looking through the internet about this and have not come up with much. Some references say not to do sit-ups or an exercise that puts downward pressure into your pelvic area, but this far out from surgery would it be a problem? I do not know. Lots of info on the kegel on the internet but mostly all variations of the same positions and movements.
Great to hear there may be hope for situations like ours, it is very frustrating and a blow to quality of life. I appreciate your help on the exercise and sharing your experience.

Ok. Here's the routine:
At home I do five sets of 10 kegels sitting down. Each kegels is 10 seconds each with a 10 second rest. If you want to take a break between sets that's fine. I do it sitting down and I time it with my phone timer. Next at the gym I do a leg press @ 50 lbs. While pushing I tighten up like a kegels then release when I bring the weight down. I repeat 10 times. Two sets here. Then I go to the hip adductor and do three sets at 50 to 60 lbs. Start at 50 and work your way up as you progress. You don't need to tighten with this exercise just do normally. Then I move on to the abductor machine and do the same. Next I get a weighted ball (about 15 lbs.) I sit with this ball in front of my chest and stand with it and tighten up down there and then sit and release. I do two sets of ten reps on this exercise. Just these exercises are enough to get you on the road to being leak free. Adjust to what you can handle and don't over do it but try to be consistent. I hope this is useful for you.

I do the exercises Monday through Friday and rest on the weekends.

Dr Wymer was very impressive,kind and I believe,very knowledgeable. I would go back to him and I highly recommend him. Also, ask for nurse Frannie when it comes time to remove catheter. A very kind and gentle lady.

I am now 10 1/2 months post HOLEP. I thought I had the incontinence completely under control, but I do not. I am finding out that any type of hot spice's and caffeine cause major uncontrollable leaks. I drank 3 cups of coffee a couple of weeks ago and paid for it for 2 days. I ate blackened Mahi mahi yesterday for dinner and have been paying for it all day today. For the past 2 months I have been wearing only one liner a day. No stress incontinence whatsoever. Now I must ensure I don’t consume caffeine, alcohol, or hot spices that can affect my bladder. I will gladly eat bland food with only salt and no alcohol or caffeine to get back to only 1 liner per day. It has been hard for me to nail down what foods, spices, and drinks cause me to leak. I must be more careful in my selection of food and drink. I was told to avoid these things on my post op instructions, but figured 10 months out I could start eating what I liked again. I was wrong!
