Aquablation: Post-surgery expectations

After decades of pills to (partially) manage BPH, I am scheduled for aquablation at Mayo JAX in mid-Feb. Has anyone had that procedure done? What was post-surgery like? What were your experiences regarding regular vs. retrograde ejaculation?

My expectations are high. My general health is good+ (71 yo), my prostate is enlarged but not massive, my PSA's suggest no cancer concerns. The surgeon expects a low-risk procedure (no incisions I believe) and a quick recovery (unless something unexpected pops up). I appreciate it's impossible to predict surgical outcomes with certainty - I would like to hear of others' experiences to help set my expectations.

Thanks, good to know. My urologist has me doing Kegel exercises, and things are slowly improving.

Day 28. Update.

So, four weeks post Aquablation.

No blood in urine, that stopped end of week 3. Flow sees to have settled at an acceptable level - not quite back to my 20's, but doesn't feel particularly restricted. Bladder muscles probably not what they were. I feel fairly emptied 3 times out of 4, but if I pause and relax I can usually encourage the last few cc's. It helps to start with a fuller bladder.

The hesitation in starting urination and pain on initiation reduced a few days ago. When I finish is stings/aches and that lasts for a while. I think this is the sphincter/pelvic floor muscles having to contract further as tissue has been removed. They are not enjoying the extra movement required. The flow is still variable, but less variable than it was.

Frequency reducing, but though I can now manage 2 hours plus, the feeling that I could go with a sense of urine pressing on the sphincter, sets in quite early. I presume just retraining my bladder and toning up the underused muscles will take time. I only got up once last night - first time - though had not drunk the usual amount during the day. I'd say I'm safe for an expedition of 2 hours between restrooms if I have managed my fluid intake.

Steady progress. It is still a bit uncomfortable to sit, particularly on office chairs and I can still feel some discomfort/sensitivity as though inflammation still reducing. There is some urgency once I decide to go, but less.

I will try to check in with a 6 or 8 week update. I have an appointment at 8 weeks and will have a voiding test and scan.

I have suffered with Prostatitis for more than 20 years. The size of my prostate has grown over the years to 90 grams. I have been getting up a
from 3 to 6 times at night. I have been miserable and have decided to go through the Aquablation procedure.
I am two and a half post the procedure, the urine flow is significantly better but I am not fully cured yet.
Since the procedure, I have been having tremendous pain right after urinating.
I spoke to my doctor, he said not to worry. I have to allow some time for the prostate to heal.
I have done some urinalysis test, the result came back with white blood cells, proteins in the urine and an increase of Leukocyte Esterase.
They have given me antibiotics for possible UTI.
After taking the antibiotic for 3 days, I am now peeing blood.
I was told that I might have some serious inflammation and suggested that I take 600 of ibuprofen and turmeric to help lower the swelling.
Any advice is greatly appreciated.

I am also getting the pain after urinating (4 weeks) and am also on antibiotics (Cipro) - which often works as an anti-inflammatory. Apparently the inflammation is slow to clear and as tissue has been removed, the various muscles are having to move further and clench harder. So like any muscle that has been unused, it complains.

Ibuprofen sounds like a good idea.

For anyone considering aquablation sometime in the future, I would suggest you also consider a PAE (prostate artery embolization). I had it done a little over a year ago and the results are amazing. They go into an artery in your thigh and up into you aorta and then down to the two arteries going to your prostate and place very small beads in them to block most of the blood going to the prostate. This is similar to placing stents going to the heart. I was in and out in half a day and had no after effects except that I no longer need to take two FloMax every day.

How are you doing over a year post op?

I still have a strong burning sensation when irritating. It’s been 3 months since my Aquablation procedure.
Currently, I am taking 600mg of ibuprofen and drinking a lot of water. I did a urinalysis a couple of weeks ago. The results came back with protein and blood in the urine. I was on antibiotics for 1 week but the pain is still there. I am repeating the urinalysis again tomorrow. Because of the pelvic pain, I have not been sexually active since the surgery. My wife has suggested sexual intercourse which may help alleviate the pain but I am afraid that the pain would get worse.
Any advice is greatly appreciated.

im. looking at redoing the rezum or doing anacquablation. i just reread the whole thread. are there any recent updates?

My husband is 11 months out from surgery, and everything is working phenomenal. All urinary symptoms gone, no urgency, dribbling, pain, retention or any others. Sex life is great no issues. I posted earlier about his recovery postop and it has gotten better and better.

I am almost 4 months out of the same Aquablation procedure. My symptoms are: urgency, burning right before the end of every urination, dribbling, retention and pain. The pelvic pain is painful at times. I am afraid to engage in any sexual intimacy. How long does it really take before I start seeing some improvement?
I did a urinalysis test which came back negative with nothing found.

I am a nurse and have kept a journal from day of surgery(previously posted)
After 3 to 4 weeks, he was symptom free completely. He had 1 postop visit, and will follow up at he 1 year mark. I hope you see improvement soon. Try to have sexual relations, might improve .

I’m retired, 66 yr old, very active cyclist, kayaker, backpacker. I had typical BPH symptoms but had not been treated by a urologist prior to a March lithotripsy for a kidney stone. The procedure disrupted such that needed a catheter until I had aquablation on 6/30. I received a very enthusiastic report from surgeon. My prostate had been 128 gms with a significant median lobe within the bladder prior to surgery. I was home the following afternoon. First night, sleep was 30 minutes at a time between trips to the bathroom. Had the expected fruit punch color urine consistently until the morning of 7/3. Still some blood at the very beginning of the urination and then it returns to pale yellow. Burning and spasms are not really an issue, I took prescribed meds the first 2 days for those symptoms then backed off to only Tylenol after that. Should I be concerned that I only pass about 200 ml between urges to pee? It’s still quite early in the healing process. I was surprised to wake up with a very modest erection the morning following my procedure. I would rate my flow as good with little to no discomfort. I find that if I wait and relax after urinating it helps to more fully empty my bladder. I am up walking around the house every hour modestly.

Aquablation day + 3 seems very positive.
