Has anyone had Prostate Artery Embolization?

Does anyone had the procedure for enlarged prostate as alternative to HoLep or Aquablation? It seems to have minimal side effects and supposedly very effective.

I had an embolization over a year ago and it didn't take. So I had a Holep six months later and have been pleased by the results.

Was there a specific reason it did not take? Was the procedure “easier” than HoLep regarding recuperation?

I had a PAE 4 months ago prior to starting prostate IMRT radiation treatment. I found the procedure to have been easy to experience. My urinary function was very good prior to the radiation.

I don't think it works with larger prostates. I read somewhere it only shrinks the prostate between 20 and 40% . So 30% is average. If you feel that would be meaningful to you then it might work in your case.

No idea why PAE didn't "take." I went to another doctor in NYC to perform the HoLEP. Of course I was disappointed to lose external ejaculations, but no problems urinating.

I think most urologists recommend HoLEP esp for large prostate sizes. It is a surgical procedure that they perform. It appears to be highly durable, lasting many years, but not without some drawbacks, such as anejaculation. PAH is not done by urologists and probably one reason they don’t recommend it. They are performed by interventional radiologists. It appears to be newer, has few side effects, works in large prostates but may not be as durable in the long term. This is why I was asking for experiences with the procedure.

Same here. It was amazing for about 3 months, then relapsed. I’m also weighing HoLEP and green light. Hate the thought of the side effect.

"The rate of complications was low for both small and large prostates. However, the need for surgical retreatment (an additional procedure) increased with prostate size, with 9% to 16% of patients with large prostates requiring additional treatment within 1 to 2 years after the procedure. This may be related to not enough energy being delivered during the GreenLight procedure in cases of large prostates."

I'm looking into getting PAE pretty soon, waiting on scheduling basically at this point. I think for many it will be a combination of something like PAE and taking occasionally dutasteride (stop the too much DHT). It is thus a combination of things. Also anything to help boost Free Testosterone, as if your body isn't turning it into Free Testosterone it is likely becoming estrogen and that furthers the BPH. Not much is scientific about the things that help convert T to Free T but things like boron, magnesium, etc are believed to be helpful. So yes, PAE if needed, dutasteride occasionally, boron, magnesium. Seems to be the way I am going.
