MRI results GP says interval micro vascular insult

I’m 43, have had 12 concussions (2x post concussion syndrome) chronic Lyme disease of 10 years, stage 2 hypertension of 8 years, had Afib for 3 years, had been experiencing massive headaches, lightheadedness, dizziness, cognitive issues, memory loss, trouble with decision making and understanding day to day simple tasks, and extreme loss of balance. Blood pressure has been 180/90, heart rate in the 40s, consistently last few weeks. Had 2 concussions within 2 months, last one developed new floater in left eye and I see “heatwaves” often. Been experiencing getting “spooked”, seeing things are not there, hearing weird noises, etc.
GP ordered MRI, the results said few hyper intense foci subcortical (deep right side of brain) compatible with interval microvascular insult without acute or active process.
GP says possible there are changes to the brain with small vessel issues, possible past micro stroke and reduced blood flow.
My neurology appointment has been moved up to May 21st, also see cardiologist May 6th,
I feel like there’s a lot at play here, none of which are good. My underlying health issues already with Lyme, with multiple head trauma and repetitive concussions, cardiac issues, etc. Am I developing early onset vascular dementia?? Do I already have it?
I have not mentioned my results of the MRI and thoughts from my GP, to any of my family yet. I honestly want to have all the information first and if it is bad news I need to process that for myself first. I’m only 43 and I can’t stand without falling over, and I start to walk sideways. My husband has obviously noticed the extreme changes as of recently. He’s very concerned. It’s alarming and scary to say the least. Any one whom has any information or similar experience please I would like to know.

I have cerebral narrow vessel disease. My neurologist said I need to keep BP under 130/. If not I’m set up for another stroke. Also, I was put on statins and blood pressure meds.

@joanm56 I’ve been on 25mg Losartan/Cozaar since being diagnosed with hypertension, but it’s been really high for over a year now, but was never upped in milligrams. Although my medical history states my high BP, I look really young, and that seems to get me in more trouble because, no one seems to me as someone with multiple health issues, so unfortunately a lot of my issues can be overlooked until it’s too late. I’m not overweight and have been very active until recently, it’s my Lyme disease that causes inflammation in my body head to toe, no matter what I do.
