Got my test results. Is my liver basically gone?

Posted by John Taylor @jdtay87, Jan 30, 2025

ALP 516, AST 97, ALT 217.

Just got these results overnight through my doctor's online patient portal. Haven't talked to him yet. Would these type of numbers warrant imaging?

@jtday87, and others... Your physician will probably send you for follow-up blood-work with some additional tests to verify the results. Also, depending on your age, other illnesses and health conditions, he may schedule some imaging. A lot depends on your condition and status...

After I receive the portal information from tests I usually go to Perplexity, I prefer this site rather than Goodgle but that's a personal choice... I go to Perplexity and begin asking questions. It responds immediately and clearly to my questions with no sponsored links or additional information I don't want to know.

I learn what I can so I can be well informed when I do chat with my physician. I want knowledge. I am my own advocate after decades of working with poor medical systems and fighting to be heard. At

My doctor's assistant called. Scheduled some followup blood work a week from now. Also said someone else would be calling me about setting up an ultrasound of my abdomen.

Hi, @loribist. I woud like to welcome you to Mayo Clinic Connect. We are patients like you and many of us have experienced the freaking feeling like you are with seeing these unexpected lab results. My experience with liver diseass and labs is that one lab result does not tell the whole story. When my PCP noted odd lab results in my annual physical, he talked to me about any changes that I had made with my medications, over the counter products like vitamins, supplements and/or alcohol, drugs. He even asked about aspirin, and told me to refrain from any of these and then I was retested. He also said that one test (the first one) was not indicative of a liver issue and that he wanted another test before referring me to a specialist.
Lori, Our doctors are the ones who are qualified to explain our lab results to us. Will you be having a follow-up appointment with the doctor who ordered these labs?

Thanks for your input and yes a follow up is scheduled next week.

Like others have posted when I was informed that my liver enzymes were off the chart I didn't know where to turn. My first notification was during a routine physical and then over the next few years a doctor who was recommended by others really had no real information about the cause. His recommendation was to avoid alcohol. Besides the enzyme levels I was also suffering with ulcerative colitis. It was only after I moved to SW Colorado that a doctor here when seeing my labs immediately sent me to a GI who had done specialized intern training with liver deseases at Mayo in Rochester. Through more testing and finally a live biopsy I was diagnosed with small duct PSC. It was I thought a death sentence since there is no pharmaceutical cure for PSC. Only cure is a liver transplant. Well, the diagnosis was over 25 years ago and now for whatever the reason, all of my liver enzymes are back in the normal. Why, no one knows. I didn't change my eating or drinking habits or my general lifestyle. So, for me I am of the opinion that I will not die from PSC but will die with PSC.

Good morning,
I would like to start by welcoming you to this group. I'm not a doctor. I am a transplant recipient celebrating 5 years. And prior to my transplant and as an ongoing situation I have primary biliary chalangas. My disease is incurable. My transplant was part of the diagnostic Care, not a cure for my disease. Elevated numbers can mean anything. Don't panic yourself. Talk to your doctor. And my best piece of advice. Don't Dr Google anything don't just look at your results and go oh self-diagnosis I'm dying of liver disease just because you have elevated enzymes. Elevated enzymes quite often mean that you have some form of rheumatoid arthritis. Let your doctor refer you to a gastroenterologist or even a rheumatologist rule out rheumatology first. Then move forward with liver issues. Maybe you have non-alcoholic fatty liver disease which by the way is 100% curable. But the best thing is don't Dr. Google. When I was diagnosed the hepatologist that I saw The first thing he said was don't Dr. Google but if you do need to look up information because we know we're all human go to the Mayo clinic site and Google through there. I wish you good luck. I pray that what you have is reversible/curable or manageable and I wish you all the best.

@lisalucier Had an MRCP and a FibroScan. My gallbladder is almost gone, full of stones. My liver has 3 out of 4 fat buildup and 2 out of 4 scarring. I feel fine though, I guess? Haven't scheduled any more followups.
