PSA detectable 18 mos after prostatectomy

My husband had a prostatectomy in June 2022. Postsurgery PSA tests were all undetectable < .10 until January 8, 2024. The PSA result was .14
Does this mean his cancer has returned? Can PSA fluctuate?
We spoke to a friend who had a prostatectomy 9 yrs ago and had two detectable PSA >.10 tests then returned to undetectable. Is this typical?

Hello Group:

I had Robotic Prostate Surgery Aug. 2, 2022. Had 5 PSA tests since surgery. Here are the levels 0.06; 0.11; 0.12; 0.13; 0.13. Met with Radiation Oncologist last week after last PSA test and he recommended that I have another PSA test in April. He said he follows the American Guidlines that which indicates radiation from 0.18 to 0.23. Has anyone experienced rising PSA post surgery and if so, what were the next steps?

Psa 1.8 then 2.2 post surgery. Got put on adt and psa dropped in 30 days undectable 11 months later. Drs want pelvic radiation therapy now.

I recommend considering these inputs. They help to explain potential next steps, when, and why. At your level of PSA, the challenge is knowing where to radiate.

Had aPSMA PeT to rule out spread or if we can find the cancer if no spread there will offer salvage radiation for cure while you’re numbers are still low good luck

Well, I think I'll be looking at next steps soon. I had non-nerve sparing RARP on 6/24/24 with pathology of stage pT3a, Gleason 9, 60% Intraductal Carcinoma, and Cribriform Patterned 4. Today, my first 3 month PSA was 0.26 mg/ml. My Mayo team says it isn't BCR until I have two results over 0.2; however, I was also told that a test false reading was highly unlikely and it is very unusual to have a reading this high this quickly - they expressed concern. My incontinence is improved since surgery,
but the team said that salvage radiation will stop any improvement and may worsen it. I'm scheduled for a follow-up PSA in 6 weeks. I know what the treatment protocol is but I also know that my situation isn't optimal. I read all the studies saying that even with high risk PC, 10-15 years was highly likely but I am also reading that early BCR with doubling rates < 6 months puts 5 year survival at < 20%. I have my fingers crossed that my next reading does not reflect accelerated PSADT. I'm a bit anxious now!

So sorry to hear this news and your anxiety is totally understandable - it’s OK to be fearful in light of your numbers.
BUT, so many advances have been made in even just the last few years! You will probably be put on some form of ADT right now to stop the cancer in its tracks and then you can exhale while your team works on the next steps.
As has been posted on other threads, radiation initiated immediately or down the line has the same outcomes so don’t sweat it out waiting for it to begin. You may be on ADT for many months - perhaps even different forms - before your team is ready to do radiation.
So many others on this board have faced similar scary situations and have come out on the other side with fantastic results and a hopeful future. YOU WILL TOO!

Need advice On 2nd Chemo of 6 for bone mets from PC My PSA has shot up to 159.3 from 62.9 in a month from 23 last May 24
Did 6 injection Radium 223 in Jan 24 start and Xtandi a year Jan 23rd Both stopped working
Still on Xgeva and Zolodex
What are my next possibilities. Getting concerned over stopping this and my overall survival

80 and feel great. Had radiation of prostate 7 years ago and clean till 2020 or 4 yrs ago

I had RP in 2008 with no follow up treatments. Eleven years later my PSA results were rising every 3 months. It was recommended that I see a oncologist and after MRIs cancer was detected in the prostate bed. I began hormone treatment for 6 months while having 36 proton radiation treatments. My PSA scores initially were undetectable but that was short lived. They're now approaching a 4 at 3.84. Three PET scans have not been able to show any cancer yet. I don't particularly want to begin treatment unless a PET scan can show me where and what to treat.
Just don't be too hasty to begin treatment. Involve your urologist and a oncologist to listen and learn. Do plenty of research.

To add to my continuing PC battle, I had a PSA screening last week with a 5.25 result. Consulting with my medical oncologist, we are still under the notion of wait and see. I'm hopeful that another PET scan will either show a metastasis somewhere so we know a method of treatment. I will have another PSA test in late November by my radiation oncologist. A PET scan in February 2024, my 4th, did not yield a location of a metastasis.
