Is a Spiculated lung nodule always malignant?

58M, Nonsmoker, No history of cancer
I went to a pulmonologist due to coughing, shortness of breath for over 2 years and got diagnosed with allergic asthma.
Ct scan showed 5 lung nodules all on the right lung.
2 are calcified -most likely granuloma,
2 perifissual- which are most likely benign
1 subpleural nodule 5mm
1 spiculated nodule 5mm
The spiculated lung nodule worries me the most
I did research and all data and studies say that spiculated nodules are malignant with a predictive value of up to 90% ..
Doctor wants to just monitor it with follow up Ct scans.
I read that in rare cases infections, Inflammation or sarcoidosis can have Spiculation but Generally such nodules are a sure sign of Malignancy.
Anoyone on here who has experience ?

I had a lung CT scan. It showed that I have a spiculated 3.2 x 3.3 cm mass on upper right lobe. Today went for pet scan. I will wait till next week for result. Yes, it's scary
Yes it’s a very scary situation and I truly sympathize with you, I know how you’re feeling to some degree because we all process things differently. I started out with a nodule that was considered suspicious but nothing was certain yet, they kept it under observation with regular ct scans for 3 years until it was definitely diagnosed as cancer…they didn’t waste time doing too many tests, instead a lobectomy was done to remove the upper left lobe. I was okay with that because I wanted rid of it and afterwards life went on. It’s easy to say don’t worry but truthfully I always worry because it’s my nature however I probably did more harm worrying and obsessing. I believe that we’re going into a new era with lung cancer because of earlier detection and newer treatments are also coming out. There are many people today living full lives with this disease and even longer, you are now in good care and thankfully they are on top of things. They will put you through some tests to determine what you need and the rest will follow, there’s many people here who have gone through this so you can get lots of information and support, I wish you the best of luck and continued success.
Welcome @jones1959, This is a scary time for anyone. Try to focus on what you know. There is something suspicious in your lung and your doctors are taking the appropriate steps. Today, we don't know that it's cancer. The PET results will be one piece of the puzzle, and additional tests may be ordered to help fill in more of the puzzle. Any diagnosis process can take time, so be prepared to hurry up and wait. Was the nodule/mass found on a routine lung screening exam? Was this your first CT?
I was just going over what I said earlier and then it dawned on me, I sounded entirely negative and that’s not what I wanted to make you feel…I totally agree with Lisa, nothing has been determined yet so please accept my apology for getting carried away. I sincerely hope that everything will work out well for you and just to add that there’s so much progress going and people are benefiting greatly from this, I keep telling myself that daily just to try and stay positive.

Had bronchoscopy came back benign

No. Back in 2019 doctor ordered a CT scan because of coughing and I smoked for years. Now in2024 , I cough a lot. Quit smoking after over 53 years of smoking,and coughing has gotten worse. I have been quit for 13 months. Then pet scan done. Waiting for results. Thanks.
what feature makes a biopsy impossible?

Thank you so much..Its impossible to biopsy due to the location in the middle lobe..I now have another CT scheduled for November as last CT showed 2 new smaller nodules and once again just want to to keep a close watch on the one of concern plus these 2 to see if have grown. really appreciate your kind words
Rats. I got my results back. Doctor calls it a mass now,not nodule. Took that pet scan and I have lung cancer. It's in both sides .
