Sinus issues causes Bronchiectasis exacebation?

Hi,
I am having another Bronchiectasis exasperation with a severe cough, along with a high sputum amount.
I nebulize with albuterol 2.5mg/3 mL nebulizer solution and 3% saline three to four times a day depending on time. I use fluticasone propionate 50 mcg/actuation nasla spray for nasal congestion, along with an albuterol 90 mcg/actuation inhaler - 2 puffs every four hours when needed.
The current exasperation has caused my pulmonologist to put me on Doxycycline monohydrate 100 mg and Prednisone 20mg. She would also like me to have a sinus CT scan and see an ENT. I've had two previous sinus surgeries with not much luck. I have a whole in my septum due I think to surgery.
I don't feel that surgery is the answer. I know the difference between a sinus infection/headache, a stress one and a migraine (had a migraine related stroke). Why have surgery when it doesn't help?
I've had Bronchiectasis since 2014 and was on an antibiotic regime for four years. I retired from teaching and was taken off of it. During Covid I taught online and didn't have any health issues.
I now have more exasperations due to being in public again.
I irragate nasal passages also.....
Wondering if anyone else has sinus issues and have found other options for relief besides surgery?
60 year old - Have found that Bronchiectasis is job in of itself!

Sorry if this is wrong place to post - a in Canada and noticed an export and FDA recall of sinus sprays today - names the brands and seems quite serious ... dont want to alarm anyone but looking for good place on Mayo connect to post or should I be concerned about spreading word?

YES! Deviated septum.
After the COVID vaccine (Moderna - December 2020) - recurrent sinus infections which lead to recurrent pneumonia. Constant antibiotics and steroids. Mine is MSSA bacteria. Tubular bronchiectasis Diagnosis April 2022 after a bronchoscopy which lead to hospitalization and Nafcillin IV. Can't seem to get cleared for a septoplasty. I have months where I need nothing - and months where I need to neb albtuerol + hypertonic 3% + Aerobika flutter valve + percussion vest twice daily. It's so strange. Tested negative for asthma. Horrible, uncontrolled allergies (living in Kentucky). Prednisone is the only thing that helps the inflammation. Also high dose Vitamin D3.

Have they tried nasal antibiotics when you have an exacerbation to rule out the sinuses as the source of your lung issues? Or cultured the sinus bacteria and the lungs at the same time?
I agree, except for fixing a badly deviated septum from a broken nose, sinus surgery did nothing for my frequent infections.

One thing that has helped me is to avoid both sinus infections ans lung exacerbations is to use Mucinex and NAC to thin all the mucus in my body. NAC is also antiinflammatory.

I use Mucinex and NAC as well as Reduced Liposomal Gluthatione for the same reason

Also tried Mullein, Quercetin, Stinging Nettle, Butterbur, Bromelain, Horehound, Marshmallow Root, Licorice Root, Ginger Root, Fenugreek, Slippery Elm, Oil of Oregano...these seem to help from the nose down to the stomach

I also take mega dose D3 with K2 and magnesium complex. Boron, Zinc, Vitamin C, Beta Carotene. It's the Coimbra protocol. Trying it so I can stop being dependent on Prednisone.

Not sure if the supplements make a huge difference, but the pulm took away a lot of my meds because I tested negative for asthma with the methylcoline challenge.

I've often wondered if I truly have asthma. When I discussed the methylcoline challenge with pulmonary they said I'd have to be off of any steroid inhalers for a while to avoid false negatives. Were you on steroids when you took the methylcoline challenge? My asthma was strictly diagnosed via pulmonary function tests. Once my lungs are clear I feel like a normal healthy person, so I question the asthma (for which I am on an inhaled steroid, Symbicort).

The Coimbra protocol sounds interesting and useful not only for the respiratory system! Good luck.

That's quite a list of things you are putting into your body! I wonder if you have calculated how much of the vitamins, minerals and herbs are being absorbed, and how many are (literally) being flushed down the toilet?

When you say you are following the Coimba protocol, are you also strictly following the diet limiting calcium intake, and drinking the required amounts of fluid? Are your kidney function and vitamin levels being monitored regularly?

While there is scientific proof of the merit of some alternative treatments, we need to remember that these substances are also "drugs" i.e. not naturally occurring in our bodies at the levels we ingest in supplements.

They can interact with one another and other medications we take. Finally, many herbs can cause or exacerbate allergies. For example, I cannot use aloe vera, in any form -it gives me hives. So I need to read all supplement and cosmetic labels carefully.

Finally, be sure you share a complete list of all of these substances with all of your providers and your pharmacist.

I understand the urgency to feel well again, but sometimes less is more. A healthy diet, adequate exercise and rest as well as someone to talk to may be more important than endless hours researching alternative treatments.

When we do choose to step into complementary and alternative choices, it is important to make sure valid research shows usefulness for our specific conditions and our providers know what we are doing in addition to traditional medicine.

I agree. I cut way back yesterday. I'm just scared. I'm so SOA. No pneumonia. Tubular Bronchiectasis. GERD under control. No asthma diagnosis. So many tests have come back normal.
Re. the Coimbra protocol, yes and yes. I also read the books on Amazon by Jeff T. Bowles. I'm stopping the Vitamin D3 for now but will continue the K2. Before that, I was following a prednisone alternative regimen recommended by Dr. Eric Berg on YouTube. This was after exhausting all options with multiple specialists. I even work at a hospital. This protocol helped me get off the neb, flutter valve and vest for months. Now, I'm back to it. So, it's time to quit.
Also, I just discovered that Butterbur is a ragweed, so tossed it (Kentuckian here). I'm wondering if Oil of Oregano is good to take to prevent infections since I seem to need a lot of antibiotics and it's not good for my gut.

D3 is considered the most important supplement for Bronchiectasis. But checking your blood levels will tell you if you need to take it or not. You want your levels to be good like between 60 and 80. You can adjust how much you are taking based on blood work. Last year my levels were 83 and I was taking 4000 iu a day. I felt it was a bit high although some Drs would say it is a good reading. So i cut back to 2000 iu a day and now my bloodwork says 63. I will continue with 2000 to 3000 iu per day based on this recent reading.
