Gross hematuria (blood in urine): Can't find cause

Posted by carolak @carolak, May 13 4:50pm

I’ve been having gross hematuria for the last 20 days. Very bright red I’ve had blood tests and CT scans,
There is no infection and the scan show nothing. Urologist seems baffled.
There has been significant amount of blood and some clotting nearly everytime pass urine.
Has anyone had this happen?
My red counts are low ( due to blood loss)

Your urologist needs to look again. It sounds like a little bleeder tucked away in a crevice. Maybe serious. Maybe not.

I agree. They only want to say nothing shows on CT scans. And they “ think” an arteriogram might show something…. But they say only if actively bleeding. And of course if it is coming from kidneys .. then which one ??? And could possibly have to test both kidneys. From what I read about this test there are some risks..
crazy and apprehensive.
Thanks for listening and your thoughts!

Autoimmune diseases like lupus can cause blood in urine so blood tests for that should be done as well. It is fairly common in females. A PCP can do initial screening with follow up with a rheumatologist if anything positive shows up on screening. Please let us know what you find out. Hope it’s figured out quickly.

A nephrologist should be consulted before invasive procedures are done. They may eventually want a kidney biopsy but will start with noninvasive tests first in the proper order.

Your physician need to put you on the fast track for resolution of your health concerns. If he is not interested in treating the mundane and common ailments have him refer you to someone who is!!!! or do a self referral! Just like everything else, they just don't make Docs like they use to. I feel I can candidly say this as having been in the Nursing profession for greater than 40 years from floor nurse to Director of Nurses. BTW They don't make nurses like they use to either!

Hi. Thank you for your comments. I totally agree that we have to be our own advocates in the health system. Ask questions.. do research… get 2nd and 3 rd opinions.. don’t just take the Drs word for everything…. I continue to push forward to find the cause.. and btw.
Most nurses are pretty darn good. Thank you for your concerns.

Thank you for that suggestion. Do you know how long she had the hematuria? Or any other symptoms or tests she had prior to this discovery. ??

Hi Carolak. I’m the person with cold agglutinin disease that sonieaml is referring to. I was diagnosed in 2015. Mine is secondary to lymphoma. Happy to snswer any questions you have about it. It’s extremely rare and most doctors have never treated or seen it before.

Hi and thanks for reaching out.
Any help and information is so appreciated.
How did they “ find” that you had CAD ? What kind of dr diagnosed it?
What steps and questions should I ask the Drs to get them to “ test” for it?
I have had gross hematuria about every 4-6 months for almost 6 years. Sometimes it lasts 5/6 days sometimes it last 30 days. It’s not a couple drops it’s quite a bit.
I’ve had CT scans .. scopes … even a kidney biopsy about 5 years ago. All have been clear. I’ve had about 5-6 different urologist over the years. Occasionally my red count will be low. Usually my wbc is low.
I have a history of breast cancer 30 years ago. And have AFIB. But that’s all good. This is just annoying and gets scary after a couple weeks of the hematuria.
Anyway. Thanks for the reach out. Hope you are staying healthy
