Optilume balloon therapy for BPH
Hi. I'm 67 with BPH and a rather small (30ml) prostate.
I've researched the new Optilume procedure for BPH but not finding others that have had the procedure or have also been looking into it. For myself I had a Cystoscopy and the urologist said ideal for aquablation I agreed to it but have since changed my mind and here's why. Unlike other surgical procedures Optilume for BPH is performed in the Drs office with only local and usually spinal block anesthesia. Optilume for BPH does remove any Prostatic tissue. What it does is a dual balloon catheter is inserted thru the urethral, it small and is inserted thru a 19fr rigid cystoscope along with the camera . The catheter passes into the bladder then is backed out so the smaller of the two balloons is in bladder and will hold the specially sized for each patient second balloon in the prostate. The balloon is then inflated causing a commisoratomy or tear between the two anterior lobes of the prostate then a drug called paclitaxel is placed for about ten minutes on the tear. Paclitaxel prevents the lobes from growing back together again. You will leave the Drs office unassisted with a catheter placed that will be removed usually in 24 hours. It shows durability in 5 year studies. Mostly for me, I have no reason to have my prostate obliterated and even partially removed. I expect to continue to have unaffected sexual function other than being relieved of the side effects of tamsulosin which have hit me hard. Orgasm quality is extremely important and seldom discussed in research. Don't confuse orgasm with ejaculation. It's the orgasm that feels good and why would any patient not preserve that for themselves. Optilume for BPH was approved approximately July of 2023 so hopefully clinics and surgeons will jump on the opportunity to provide a very effective alternative to tissue removal.

I had bloody urine about a month ago. Went to local ER. 7 hours later they decided to stick a catheter in me. I overheard the nurse and EMT talking about how they could not locate any lidocaine lubricant, it was hard to get. Something was rubbed on the end of my meatus (see how I've learned to talk medical lingo). I know how much it hurt when they put it in. I was sent home along with 2 bags. The next 6 days I like to call "my week in hell". The pain was terrible. I finally went to my PCP to get it removed. What a relief. I think some kind of damage was done. After a month, I am just beginning to get my normal urinary schedule back.

It's now been two days since I underwent the Optilume for BPH procedure. I have no Pain whatsoever. I have had bladder spasms because of the catheter inserted at end of procedure. I removed the catheter myself with no pain and was able to urinate one hour later followed by bladder spasm much less intense that when the catheter was in. I'm amazed that there is no pain. I have taken one oxybutinin the night after procedure to help with Spasm also Lorazapam twice daily for three days. I have had no need for pain medication at all. I really had no spasms except when I was up to have a bite to eat while the cath was still in. I am unaccompanied in my recovery. Drove to my daughter's house yesterday to feed the cat. Will kick back and relax now for a few days for the prostate to heal. I've urinated three times today and without pain and clear of blood. I am taking AZO
medication for urinary pain that turns urine orange I need to check how long I'm supposed to take that. I have resumed my regular diet and all systems are go. It will hard to keep me down as I do keep myself busy. Any questions? Happy to answer.

Day three after OptilumeBPH.
Because I can't take NSAIDS to reduce inflammation I'm not at full flow and some blood has shown up in my urine. Still no pain whatsoever. As the swelling of the prostate slowly precedes so will my flow increase. Bladder spasms have gone away. I'm no longer on AZO and Lorazapam. Feeling pretty good this morning so will go about my routine but no strenuous activity. Everything is going as I expected or better. No regrets. Happy with my decision. I'll continue to update.
Hi. Short term just to heal. Full flow will occur after swelling. I've had no pain at any time. Long term 5 year results are out and showing this to be a durable option far less complicated than anything else. I thought it would have some pain bu none surprises me.

Day four. Still amazed that I've experienced no pain at all. Still passing blood clots but very few ans smaller. Flow is in creasing so swelling is subsiding. Had a full schedule today. Spent a couple hours trapesing around a nursery selecting plants, out for lunch and drive about the countryside. No urgency while I was out. Great day.
I had optilume treatmen for bulber stricture 2 months ago. Stricture was due to procedure called
green light photovaporizatin for BPH.
Despite 4 months post, I am still having episodic hemeturia with either brown clots and times fresh clot.
Repeat culture urine tests are normal.
No UTI,kidney issues.
I an 66 year old male.
Any one please provide insights, guidance.
I am getting frustrated with this ordeal.
Please help.
Any updates on Optilume?
I'm in my mid-sixties and thinking of getting the Optilume procedure for BPH. So far, the medications I have tried have either not been effective, or have had bothersome side-effects. The advantage of medications over procedures is that I can simply stop taking the medication if I do not like it. I'm worried that a procedure cannot be undone. I'm particularly concerned that a procedure would affect sex, including the enjoyment or orgasm.
Anyone have any feedback?
I am 78, very active and healthy. Had years of urgency, weak stream, multi nighttime trips to the toilet, never went anywhere without first concern “where’s the nearest toilet.” Took finesteride but could not tolerate flomax. Turned down TURP offers many times as too risky for my taste. Three months ago had PROSTATE ARTERY EMBOLIZATION. All previous symptoms already gone, pee like I am 19 again. Easy procedure with zero sexual or other side effects. Best thing I ever did. I believe this new procedure done by a radiologist will be the new BPH treatment Gold Standard and put urologists and their scalpels out of business. You definitely should check it out!
