Surgeon missed the margins. 2nd surgery

Posted by freebird67 @freebird67, Apr 1 6:16am

Good morning all,
Had lumpectomy 3-18 went back on 3-28 thinking surgeon would release me to oncologist but No ! She told me they missed the margins. I was kinda dumbfounded at first, she explained to me that they have to go back in and cut more and scrap. Then asked me.... do you have a problem with this ? Of course I have a problem with this, who wouldn't ! She just told me cancer cells were pushing on the margin. So I'm headed for another surgery this Friday.
I'm loosing faith in the medical system fast. The nurse came in to schedule my surgery and said it happens like no big deal.
So I guess my question is, have any of you gone through this ? Did it change anything for you ? I did do some research about it and didn't like what I read. But have no choice on this. Thank you and
Have a blessed day

Did they do genetic testing on you in the beginning? Are you saying that you had lymph nodes involved but were not told? Prayers that CT scans are clear.

@sirene
Yes, I had genetic testing before surgery so we knew I was BRCA2 positive which is why I opted for a double mastectomy and ovaries/tubes out.

LVI is not the same as lymph nodes, but maybe as significant. The way I understand it, it’s cancer cells in the blood vessels in and surrounding the tumor. My lymph nodes were clear. Not everyone with cancer in lymph nodes has LVI according to a study I read. There are different schools of thought on using LVI to determine treatment but there are studies indicating LVI leads to greater risk of recurrence and metastasis and a poorer prognosis. Many believe LVI requires additional treatment. LVI was never discussed with me as a risk factor, but I now see LVI in the breast oncology surgeon’s notes from 2013. I think my case supports that LVI is significant.

Thank you for your prayers.

I am so sorry that you are having to go through this. I apologize but what does LVI stand for? Lymphovascular invasion? I am going to go back and look over my notes.

Yes, LVI is lymphovascular invasion.

My report does have LVI listed but I can't tell if that is a negative sign after it or a hyphen for the DCIS. I am messaging my doctor about it. Thank you for telling us about it.

OSF: Left Simple Mastectomy with SLNB:
IDC, G2, forming 2 masses, 14 x 11 x 6 mm at 12:00 (first) and 4 x 3 x 3 mm at 2:00 (second); LVI-. DCIS, multifocal, solid type, G2, present at 12:00 mass, 2:00 mass and 2 x 2 mm lesion (third).
- Superior anterior surgical resection margin positive for IDC at 2:00 )second) mass.
-All surgical resection margins negative for DCIS (closest margin 1 mm posterior).

So sorry my mammos did not show my IDC cancer at all. If I did not feel a rock lump I probably would be dead. I had to show them with my finger the lump because ultra sound swiped over breast but not the spot. You have to be your own advocate.

On July 25 I am having an mri with dye. I want to have an infusion wash the dye from my system. I have read the gadalinium(sp?) Goes into brain and other organs. Who has had infusion after mri? Does my doctor order it to proceed mri? Do I request? Please respond if you have had infusion. Thank you

This is why I just had a double mastectomy and called it a day . No problems since .It made me too nervous to worry about any risk of further growth of cells in the future . It is an individual decision as many women want to keep their breasts . I was 59 at the time.

Glad to hear you are done. Just had Double mastectomy with reconstruct. DCIS grade 3 - no hormone receptors. They are referring me to radiation onc because margins were clear but close. Appt with onc next week. Really don’t know what to do.

Who has had infusion after breast mri with gadalinium dye. The infusion washes away as much as possible the dye. The dye stays in system and not good.

It is only natural to worry about this. Drink a lot of water and other liquids after the MRI. This will help to flush all the gadalinium out of your system. That was recommended to me by the medical personnel. However, I don't know about any infusion. Maybe your doctor will have a recommendation. Researchers will have studied this extensively, to make sure that the benefits from the MRI will outweigh any risks from the gadalinium dye. I recommend discussing your concerns with your doctor, or other medical people who might know something about it.

@lislordav, how are you doing? Did you meet with the radiation oncologist? What's next for you?
