Useful tool to take to your next doctor appointment

Thanks to C.R.H. for posting a useful tool on Facebook — Mac Lung Disease Group. It helps to record symptoms, set goals, questions to ask, etc to prepare for appointments with NTM specialists. I’ll be printing a copy for my next appointment.
