worsening impacted stool, enlarged abdomen

This problem has persisted for over six years, despite major efforts using a variety of laxatives. Treatment began with non-prescriptions, which worked for a while, then didn’t. I tried several prescription drugs, among them linzness, trulance, ibsrela, and Motegrity. All had no effect. Condition is now worsening again. Has anyone solved this problem? Was the ultimate treatment an operation? Any help appreciated. It took a few years to be diagnosed--at first doctors insisted it was stomach fat and suggested exercise.
