Who is a good candidate for Intermittent ADT?

Who is a good candidate to take a holiday from ORGOVYX until psa start rising and then go back to taking ORGOVYX again?
Is it only applicable for non-aggressive cancers or High Gleason score or metastasized cancer?

@psmick Did you have your blood draw yet (5 months since Nov 2024)?
I was wondering if your are taking (or will take) an ADT holiday. I was Gleason 7 (3+4) non-metastatic two months ago, been on ADT Orgovyx (my 4th week now), and just completed today my 5th and last session of my SBRT treatment. My oncologist indicated that we'll see if I can be off Orgovyx after 8 months or a year. I think it will help if I compare notes with you, since we are both Gleason 7.
@web265 You were also Gleason 7. Are you still off ADT?
@linric70 How is your husband now, still on/off Orgovyx, works well for him? What was Gleason score and PSA when he was diagnosed. I was Gleason 7 (3+4), PSA 10.06

My most recent blood draw was in March, with testosterone increasing from 10 to 119. My PSA also rose from .01 to 1.6, with my last Lupron injection on August 14. My Urologist has given me two more months of AS, and wants another blood draw in May, at which time he/we will decide how to proceed. ADT is the only treatment I have received for my non-metastatic cancer.

Correct me if I am wrong but you had no metastasis on imaging and are 89 years old. On a scale of 1 to 10, how well are you coping with the ADT side effects?

I would think you are the poster child for an ADT vacation but ultimately it is your call.

I am sure if you are referring to me. Other than fatigue, I don't have any visible symptoms. There are some negative changes in my bio markers. I aged about 6 years more biologically, since I had been on ADT(one year), according to Function Health! I take that with a grain of salt!

Meant "I am not sure". Definitely ADT had some cognitive effects, as I am doing these mistakes more often than before!

Yes, I was. Why are you on ADT? Two cores at 4+5, right? At 70 or even 80, it makes sense but at close to 90 it must have been a tough call to make.
Not sure you will get a lot of guys with similar experience to help you but you are obviously a pretty strong fellow. I wish you the best.

My PSA was 9.6. Urologist ordered an MRI. Results PIRADS 5. Now I am stuck with Biopsy on March 5, 2024. Then Gleason 9 but localized. I just did Grand Canyon Rim to Rim Day hike 5 months earlier. I was pretty healthy and fit.
I had no choice but get a treatment as advised by my primary care physician and urologist.
Had I not tested for PSA, it would have been a different matter!

So you were fit and healthy 5 months ago but not so much anymore? I don't understand the rationale for the ADT given confined to prostate and your age. Can anyone else here explain that to me?

Still off ADT for now. Stopped taking Orgovyx on 6/1/2024 PSA has been < 0.006 since that time with the lastest test being done 2/28/25

PSA was 77 (2 years ago), Gleason was 8.
He has been off from Oct. 8th until Feb. 28.
Started back on Xtandi & Orgovyx on the 28th. He had total knee replacement on March 4th. I was worried he'd had the bad hot flashes and night sweats after surgery but it went great.
He has not had surgery because it has migrated to a lymph node and a spot on his hip bone.
He is still under a 6 week watch. Blood draw, then follow up - above 2.0 on drugs, below 2.0 for two cycles vacation. He's now reading a .22. This has worked for him for a little over 2 years now. I still hold my breath every 6 weeks waiting for PSA results.
