Where to start for newly diagnosed advance prostate cancer

Posted by wg2023 @wg2023, Feb 28, 2023

My dad 82 was diagnosed last week with prostate cancer with high PSA 100 and GLEASON: ( 4 + 4 ) = 8 / 1 0 , he had the
PET Ga-68 PSMA image Friday and today we found out the result is stage 4 Metastatic lesions in the bone pelvic area.
They want him to start radiation 25 sessions and chemo pills of Casodex 50 mg 28 pills. For now and mentioned an injection on the last day of radiation.
I’m lost on weather this is right for him we’re looking for second opinion he is currently overseas (Turkey )and this has caught us all off guard.

The doctors where he is asked to start treatment immediately and I agree but I’m still looking in bringing him here Any recommendations to where I should take him herein the US. I’m in the DMV area Washington DC and open to suggestions based on your feedback it help direct to the best place he could receive the care he needs. For his condition.
I have read a couple of posts and interested in the new drug Pluvicto (Lu 177) would this have a special requirement to be eligible for treatment or is it available upon prescription to patients who need it as it’s relevantly new I’m not sure about availability?
I highly appreciate feedback and recommendations. .. to get the ball rolling
Thank you for reading

I hear Pl 177 is only after you exhausted all avenues Its for palliative cases

Im on Zolodex injections and after 2 years added Xtandi or Enzalutamide

Johns Hopkins is a center of excellence. Campuses in Baltimore and Washington.

Best wishes to your father and to you.

Pluvicto was okayed by the FDA in March, 2022 only after a patient has tried chemotherapy (per my doctors at Mayo in Rochester). Someone on a different thread said their father’s doctor at Mayo suggested a treatment in Turkey that isn’t available in the US. This was in the last couple of days.

I would advise Sloan Kettering in NY or Mayo/Rochester along with Johns Hopkins.

Thank you so much I will try to find that comment and see if I can find where in Turkey.
And maybe try to reach out to Mayo in Rochester if possible
