What was your highest PSA and still have a clear PET/CT scan?

I just got the results from my most recent PET/CT scan;
"There is no abnormal PSMA uptake to indicate recurrence or metastatic disease anywhere despite the rising PSA."
12/11/2023 PSA 0.02
1/5/2024 PSA 0.08
3/11/2024 PSA 1.02
6/10/2024 PSA 6.67 (This assay was performed using Beckman Coulter reagents and test kits) I'm not sure what that is???
Background; RRP 3/25/2021, 34 rounds Salvage radiation fall 2021
11/11/2023 PSA 6.56 PET/CT showed metastatic lymph node in chest
3/14/2023 began 6 month MetaCure Trial Orgovyx, Erleada and radiation to the lymph node. PSA was < 0.01 at 6 month point when the drugs were stopped 12 weeks later 12/11/2023 PSA was 0.02.
I am on active surveillance, no current treatment. PSA has continued to go up. My last 12 week PSA test was a 5.55 increase.
I have an appointment with one of my Oncologists next week.

I went in for another PSA test today before my tel-med with my oncologist this Thursday. It showed another steep increase in six weeks it went up 4.09 points. From 6.67 to 10.58.
My PSA results this year;
01/05/2024 PSA 0.08
03/11/2024 PSA 1.02
06/10/2024 PSA 6.67
07/22/2024 PSA 10.58
PET/CT scan on 07/11/2024 results "There is no abnormal PSMA uptake to indicate recurrence or metastatic disease anywhere despite the rising PSA."
I am very confused and getting concerned about whats next...

I had a similar experience to yours when I recurred in 2018. Rapid doubling time but couldn’t find on scans. PSA got up to 5 without finding anything. The opinion was that the cancer was small bits, scattered around. Each bit too small to show significant uptake. I did PSMA and choline PET scans.

Not all prostate cancers are PSMA avid. There other PET scans that do not depend on PSMA.

My most recent PSA was 10.58 up about four points in six weeks. Recent PET/CT scan was clear.
I had a virtual visit with my Oncologist yesterday. He said that if the scan had shown metastases then they would have a clear idea of where the cancer had spread and that they would be able to do a more targeted approach to treatment. But that was not the case.
So now I will be resuming Orgovyx and this time I will also be taking Xtandi instead of Erleada.
When I was on Orgovyx and Erleada last year my PSA dropped from 13.46 to 0.15 in one month. After three months PSA was < 0.01. PSA It remained undetectable for about three months but started to climb once I stopped taking the drugs. It continued to climb for 7 months to where it is now 10.58.
The side effects I experienced with the Orgovyx and Erleada combo were tolerable but I am not looking forward to living with them again. The side effects were; hot flashes, loss of muscle strength, loss of bone density, loss of body hair, loss of libido, increased breast size, and the worst one for me was joint pain.
I expected the treatments would resume and that the side effects would be similar to last years. I live in a 140 year old general store building with the main living space on the second floor. The side effects of the drugs last year made it painful to navigate the 21 steps up to my apartment. So over the last few months I have been renovating a small apartment on the first floor of my building, just in case. I am very happy that I decided to do the renovation, I may be living down there sooner than later.
