Walking a tightrope: Gleason 9 and bilateral kidney stones
I was diagnosed with Gleason 9 in August 2024, 6 cores negative, 1 core Gleason 7 (3+4), 3 cores Gleason 9 (4+5), 2 cores Gleason 9 (5+4), all negative for perineural invasion. Been on Orgovyx since August 2024. Latest blood work results, last week: PSA less than 0.04, testosterone less than 1. Only side effects from Orgovyx are occasional brain fog, loss of libido, ED, genital shrinkage. Been exercising for two hours or more daily, since the 1980's. Feel good physically. Latest XRAY, two weeks ago, showed an 8 mm kidney stone right midpole and a 7.5 mm kidney stone left lower pole. Been drinking a lot of water, but not in the mood to undergo lithotripsy to crush the damn kidney stones while dealing with PC. Any suggestions, feedback, etc greatly appreciated. Thank you!

I have Gleason 8 (4+4 and Gleason 7 4+3 ) and 2 lymph nodes..
PET scan showed: Kidneys: Small, non-obstructing kidney stones ("calculi") were noted in both kidneys (4 mm on the right, 7 mm on the left) [1]. These are generally common and often pass on their own, but can cause pain if they move [1].
Mine are a bit smaller...if yours are not bothering you, then I would continue to flush with lots of water, lemonade etc...and if they are not bothering you, then hopefully they will slowly dissolve...!

1 ReactionJust so you know, the only Gleason score that matters is your 5+4, You are a 5+4 Gleason 9 and any lower numbers are irrelevant. The top number is the only thing doctors care about.
You talk about not getting your kidney stones crushed. I’m 78 I Run on a track a mile twice a day every day and go to the gym three times a week to do weight exercises.
I’ve been on ADT for almost Eight years (Took an eight months break or it would be almost 9 years).. I’ve had two knee replacements and one hip replacement during that time. Those are much more significant surgeries/procedures than getting your kidney stones crushed.
I’ve had brain fog, loss of libido, ED, genital shrinkage going on 9 years. I just had an abdominal surgery to remove a 7mm tumor less than two weeks ago (not PC).. I’m still walking on the track Twice a day every day instead of running for three or four weeks Because they had to open up my stomach, Couldn’t do it arthroscopically.
And you’re telling me you won’t get to kidney stones crushed. What’s holding you back?
There’s no reason you can’t do it.

I’m not in a rush to undergo lithotripsy at this time because these kidney stones are non-obstructive and also because my 2005 experience with lithotripsy and subsequent recovery was highly unpleasant.

Every time I hear about kidney stones, they are excruciatinglypainful. Interesting point, non-obstructive.
Took me a year and three months before my left knee was completely working and pain free, now that’s what I call highly unpleasant.
@jeffmarc I have had 3-4 bouts with kidney stones..started when I began using creatine supplement in Houston in late 1990's ( did not hydrate enough) ..first time was the worst..my exp was it made me feel VERY ill, nauseous, uncomfortable..couldnt sit..and I did not have a clue as to what was wrong..spent 3 hrs in ER pacing and waiting for doc..he diagnosed it immediately..I had litho a few days later and eventually ( 2 weeks later) passed a stone about the .22 cal size ( less brass) ..that part was painless..the other times were similar symptoms tho not near as bad...but pain wise, maybe 6 or 7 but you know there is something very serious because of nausea...I never have been a green beret with drinking alot of water but since starting radiation and having to keep bladder full, I do much better..and drinking 1/2 litre of water each morning does make you feel remarkably good !
