Using Decipher GRID report to help guide your treatment decision
I'm 67 years old with a prebiopsy PSA of 7.8, three mpMRI PIRADS lesions of 3, 4, & 5 and my MRI fusion biopsy found 5 cores of very low volume 3+3 and 2 cores of low volume Gleason 3 + 4, out of 15 cores. I was considering Active Surveillance (AS) and requested a genomic test. My urologist suggested Decipher. My doc gave me the standard 3 page Decipher "Patient Report", which was helpful, but someone in this support group suggested obtaining the more detailed Decipher GRID report.
I called a Decipher representative and received my GRID report, in 15 minutes, by email - and am I glad I did! My diagnosis was "borderline" for AS, so the additional GRID information helped to confirm this treatment decision. Page 2 of the GRID report includes an easily understandable "Clinical-Genomic Risk Model" (see attachment). Essentially, the combined model helped to confirm that AS is a viable choice for my diagnosis and my urologist agrees.
Has anyone obtained both the Decipher "Patient Report" and Decipher GRID report? What was your experience with the usefulness of this information and did it influence your treatment decision?

I will ask for mine now that I know it exists.
I am on AS with two lesions 4 and 9 Pirads 4. PSA in August was 5.9
I just called them at left a message on voice mail. Additionally I sent an email last Friday. ........ So far crickets.....
...maybe closed for MLK holiday weekend???
Anyway, here's the contact info of the person who helped me get a copy of my GRID report: Michele Abolafia , Manager Customer Experience, michele.abolafia@veracyte.com, 858.322.6129.
You will have to call to speak to Michele (or someone), during normal business hours, since they must send a release form, to your email address, for you to return via electronic signature. In addition you will have to send a scanned copy of your driver's license.
That said, Michele told me she would be looking for my completed form while I was still speaking with her, which I was able to turn around in ~10 minutes. I sent my scanned drivers license directly to her email address, along with an email address they specified....privacy is (obviously) a key concern....
Of course (hopefully), the doc who ordered your original 3 page Decipher Report could also obtain a copy of your GRID report. However, I would be surprised if your doc is going to be willing to take the time to review the intricacies of your GRID report with you...again...it's research use only and contains a boatload of data.
The GRID report is a detailed 7 page report and every page has the heading "Research USE Only" (see below)...this is important to keep in mind. Veracyte has been collecting data since 2015 and they now have >140,000 whole transcriptome tumor profiles. Apparently this latest Decipher GRID (for Research Use Only) database uses a proprietary AI platform to compile and generate a report that is personalized and based on the genomic test results obtained from your particular cancer cells, as compared to all the others in their database.

Among the boatload of info in this report, I found some intriguing stuff regarding Gene Signatures related to "Tumor Metabolism" (i.e. scoring based on Androgen Signaling, Cholesterol Pathway, Estrogen Signaling, Fatty Acids, Glycolysis). I'm still trying to understand what exactly this may suggest...
In any case, this is undoubtably much more information than most will want to know and I'm sure most docs will roll their eyes if you ask specific questions about the intricacies of your GRID info...but for us "data nerds" its where all this is heading regarding individualized treatment plans....for better or worse.....

1 ReactionI contacted the decipher people and they told me my doctor has to request the report before I get it. I will contact the person you listed to see if I get a better response

Thanks for the link to the video. I had Decipher run on my biopsy sample in October 2018 and it came back with a jaw dropping score of 0.94 (yikes!!!). I also requested and received GRID results back then. Do you know if they can rerun or update GRID results since it is research only information? My thought is that if they do fairly comprehensive sequencing and not just targeted sequencing, maybe there's new information available.

I would think one’s score would not change, but maybe more research based interpretation is available than a report provided years ago??? IDK…
In any case I’ve read a paper where a 2nd Decipher Score was run on a second biopsy…
