Unusual case of bronchiectasis

I am wondering if there are any others out there with a similar experience: I contracted TB at age 19 during a trip to India, which was misdiagnosed at university for almost a year. I developed a moderately advanced case of the disease and even a cavity in my upper right lobe. Fast forward to 2012 and at age 60 I had a massive hemorrhage from my bronchial artery that almost killed me. No bleeding subsequent to that save for one instance in 2020 when they were using Lysol and bleach at my mother's during pre-vaccine Covid. I only coughed up one glob of blood and it stopped. Now, on October 31, 2023 (turned 72 that September) I coughed up about a teaspoon of blood, had a CT scan, and I had no ruptures. HOWEVER, I have continued to have episodes of coughing up small amounts (teaspoon +) of blood since then on at least ten occasions. Finally went to emergency room on Feb 29th, had another CT scan that showed no changes since November, or even since a year before.
Since this scary pattern has started, I eat pureed soups almost exclusively and about 5-6 x's a day b/c I find that I get congested far more easily by foods than I used to and am afraid to cough for fear of triggering a rupture. I can no longer sit down and have an ordinary meal with solid foods. I have been predominantly vegan for some years, so I eat no dairy of any kind. Yet the propensity for congestion has gotten far worse than even last September. I control the congestion quite effectively by having warm water with me at all times and drinking it immediately when I feel an obstruction and a cough coming on. It miraculously dilutes the secretions and the urge to cough goes away. Sometimes, I may have to drink 12 ounces of water at once (rarely) but that does the trick. So what I am navigating is this tricky space of balancing clearing of secretions in a gentle enough way that doesn't trigger a bleeding episode--or at worst a massive hemorrhage that could kill me. Anyone out there who can relate to this?

@paxmundi Welcome to Mayo Connect. I am not sure I see in your history where bronchiectasis was diagnosed, but it would not be that unusual after the lung damage caused by TB.

I understand your concern about hemoptysis, and wonder if you have recently seen a pulmonologist about it or your increasing congestion. Even though your CT didn't show changes in your lungs, it sounds to me as though you might have an infection in your lungs.

People with bronchiectasis are susceptible to a number of lung ailments including Non-tubercular mycobacteria (MAC/NTM), pseudomonas, various fungal and others. The usual method of diagnosis is to take a sputum culture and see what grows - these do not show up with a simple blood test.

Have you had a sputum culture to see what might be growing in your lungs?
