Under what circumstances is it OK to treat Gleason 6 ?

There is no evidence that true Gleason 6 will ever metastasize. Shouldn’t all people with Gleason 6 go on AS?

What are your thoughts?

Michael thank you for sharing and your encouragement. I never knew this would have the emotional impact it does on me . Being one who doesn’t usually show emotion my eye have been opened, be it only a few times, but very surprising to me. Back to original post 3+3=6 may not need any action or in my case, action, RP . In 7 days I will have more definitive info (was I just a 6, or is it a higher Gleason) just under rated. I do come from the belief if a sticker is in my hand get it out so it heal and stop hurting. Still no regrets.

Almost 9 weeks after surgery now. First psa was 0, will be checked once a month for 3 months. Do well, pull up at night but almost dry in morning. One lighter pad in day, I only leak 1 to 2tablespoons in 24 hours, getting better and better. Sex with pills and pump but hope to be just pills in future. Up to 40% without pump. Prostate biopsy did upgrade my 3+3 to 3+4 but all confirmed contained in prostate. Every once in a while I have a Pitty party at night and tear up but soon realize just how Blessed I am then back to the good life.

We were told by current biopsy results that my S.O. was a 3 + 4 = low 7. After RP at our request, 10 days ago, he is actually a 4 + 3 = high 7, along with some other surprises, including extensive intraductal carcinoma (IDC-P), extraprostatic extension, cribiform pattern and perineural invasion. He is staged at pT3aN0.

With those previous biopsy results we were told he could remain in AS! Thank God we decided to be proactive. He had hidden poor prognosis factors that no one knew about. This is a top notch University Hospital with the top surgeon and team.

Now wish we had removed it back at 3 + 3. Just our experience.
