Undecided choice of drugs for Osteoporosis

Posted by peace44 @peace44, Oct 10, 2023

I am 80 years old women and have borderline osteoporosis which affects my entire body. My doctor wants me to take Prolia and the Rheumatologist wants me to take Reclast. I’m unhappy with both choices as the side effects are great and I do not tolerate new drugs well. I am very undecided and am considering doing not taking annty drugs for this condition. I have never had a fracture and I’m very active Any opinions or advice.

@normalhorn both my docs say there is no data on Evenity after Forteo or Tymlos. Can you share a link?

I am doing it anyway. I sent anecdotal evidence to my doc and he finally wrote "thank you for your persistence" which made me smile.

From the former Physician's Desk Reference. You have to go to Dosage and Indications.

I’m at Mayo now for my yearly physical and found out my bone mass in left hip had reduced rapidly within the past year, Mayo has been watching it through bone density test for several years. After my scan I was told I needed to start taking Fosomax and I was so concerned about side effects. I researched alternatives. After another conversation with my Executive Health program doctor he made an appt with the endocrinologist here at Mayo. It was a great idea and I am now at peace with what I need to do. She is allowing me one year to try and rebuild bone mass through lifting weights, more cardio, healthier eating and taking better care of myself. Also, I will start the supplement Algae Cal. I’m just not ready to settle on taking Fosomax. Do your research and if you have issues such as me, I encourage you to see an endocrinologist at Mayo. I am praying the life changes will make the different to me. I realize its all up to me.

guinivere, good answer, "no." Reclast isn't the worst osteoporosis drug, but why not take the best, Forteo. I think that is what McCormick recommends and what he has taken. Though it's probably time to visit your brother, I think you can gather as much from the McCormick videos.
I like Forteo and have been taking it without struggle for almost a year. Some have adverse reactions so you have to be ready to switch to Tymlos. I hope you get a good endocrinologist and the best medication. Oh, Forteo helps heal the broken bone. Reclast does not.

What are you going to take after two years on Forteo? I am afraid of Reclast but I have read that Fosomax, which I tolerated very well twenty years ago, does not work as well.

What have you decided to take after Tymlos? I stop Forteo in July. I see Dr. Mannstadt in May, one week after my bone density test.

@janflute I am doing Evenity for a few months, and then Reclast. I cannot take Fosamax due to GERD. You could try Reclast (even a test dose, if your health justifies that) and then go to Fosamax if you cannot tolerate the Reclast. Most people seem to do fine with it and it seems- your doc can confirm- that if you tolerated Fosamax well then you have a good chance of tolerating Reclast. Reclast is more potent but McCormick and a mentor on here have described taking Fosamax after Forteo or Tymlos, with adequate benefit.

@destinnana if you decide to start meds, ask about a bone builder rather than an anti-resorptive. You can watch a YouTube video by Dr. Ben Leder on Combinations and Sequencing. Most docs say that it is better to start with Forteo, Tymlos or Evenity and then "lock in" gains with a bisphosphonate like Reclast of Fosamax (or Prolia, which many of us avoid due to problems getting off).

It sounds like your scores aren't too bad. Would you consider a consultation with Dr. Keith McCormick who advises on on-medication approaches ("whole body") as well as meds when needed. He is online at osteonaturals.

Thank you so much for your response. Why did you decide to take Evenity for a few months and then Reclast? I remember that you said that you could not tolerate Fosomax.

Tymlos gave me excellent gains. But my femur neck was still -3.6. Both my endos (two different practices) have never done Evenity after Tymlos but agreed to let me try it. One doc said this is my very own protocol. Not suggesting it for anyone else- I don't know the results yet!
