Unable to assert myself at 73...I have Severe Osteoporosis
I am on Prolia for Severe Osteoporosis but needed tooth extraction traction which can cause ONJ so together with that problem I also have EXTREME DENTAL PHOBIA so I had an appointment for a Consultation with a brusque and no nonsense Dentist albeit extremely good at his job he gave me an appointment for the extraction and told me I would lose all my teeth due to Gum disease but I need to have my teeth cleaned every 3 months and to go downstairs in the building nd get them done but I refused as I am too nervous...but now I am trapped ...I need the extraction and Dentists are rare and hard to come by..I am 74 and still as meek as I was as a teenager..Dentists are really expensive and hard to come by and this Guy knew more about Prolia and the dangers involved than my Doctor he told me ONJ was not rare as my Doc told me she knew Zero about it but im back on the 8th January for the EXTRACTION and then the cleaning which terrifies me as it takes an hour....SORRY FOR GOING ON BUT IM SO WORRIED

are u sure u even have gum disease? during covid my dentist of 25 years decided to retire, i found a new young dentist near my home, on the first and only visit, he said i had Pyria and i needed to start immediately on doing a deep cleaning of my teeth, i had heard this was painful, also i didnt think i had any kind of gum /teeth disease going on. i had went to get my teeth cleaned, i didnt have any problems with my teeth. i asked if i could just get a routine cleaning anyway, and he refused to do this. i found another dentist who said i didnt have any thing wrong with my teeth at all and didnt need this expensive treatment. im the same age as you.

An idea: next appointment, if you find that your assertiveness is flagging, imagine that all of us here are standing behind and along side you and behind you for added strength. Good luck and let us know how it goes.

8 Reactions@stanfojc unfortunately my GP will not give ...she is new and no understanding of my predicament..I am in Ireland and we have catchment area Docs not easy now to change Docs and at nearly 74 I find it too much hassle I told the Dentist I had extreme phobia and in the Middle of the Consultation he asked me to get Cleaning done straight away..mine is not the pain anxiety but the feeling of not being able to breathe....😥

2 Reactions@loribmt Lor I just won't do MRIs I am so claustrophobic that is my issue with the Dentist I recently had 2 hr surgery for skin cancer and only had local anaesthetic and was fine..I feel I cannot breathe when they put stuff in my mouth....I ate chocolate toffees over the Christmas the " Hound that I am " calling someone a Hound in Ireland means greedy anyway I have been dying ever since ....again we over exaggerate here " Im Dying " can mean hung over or just amused...the Dentist did give me Antibiotics to start 3 days before...the only plus is he does know his job..he said Happy Christmas to me before I left and I just stood there in shock before falling out the door..I obviously didn't make a good impression....but I was thinking of asking for earlier appointment because of pain now....an example of my low self esteem when in Scotland as a young one wearing open sandals on a Bus someone stood on my toes which was quite painful and I said I was sorry ...I kick myself for that...but I am in a bad situation with the Prolia..I am trapped so I have to get this done....il let you know if I am alive...ignore the drama in my comments I do have a great sense of humour...I wish the Dentist was a fan..🙈

1 Reaction@laura1970. I think it’s really a good idea having an appointment to discuss your medications and the upcoming dental extraction. Hopefully your dentist can reassure you that the potential for developing ONJ after the extraction is very rare. There are precautions taken prior, during and following the extraction to minimize the potential for infection.
The first priority post extraction is to keep the blood clot intact. I mentioned the preventive steps for that in a previous reply. ONJ can develop if the bone is openly exposed after the extraction with no blood clot or tissue covering the bone. In that situation, blood can’t reach the exposed area causing bone cells to die.
That’s why it’s crucial to make sure the blood clot isn’t disturbed once it forms. You can develop what’s referred to as a dry socket. That’s infrequent but if it occurs, it tends to be a lower molar area. The first indication of a dry socket being the extraction site might feel better for a day or two and then become painful again. You may have an unpleasant taste and/or odor.
If that happens call the dentist to be treated appropriately with the next steps…which generally are to rinse the site, pack the opening with a medicated iodoform gauze or other medicament to protect the bone while healing or if needed there may be a prescription for antibiotics.
I’m not sure where you’re getting the information but meds are effective in treating ONJ. There are 4 classifications of ONJ each with their specifics for treatment.
The statistical risk for developing ONJ for a non-cancer patient, as of August 2025 is 0.7%. That’s very low and should be pretty reassuring for you!

Please let me know what you find out with your discussion. Have a list of questions ready for your doctor. I really feel that you’ll have a better understanding that the risk is low for ONJ. The greater risk is not having a healthy mouth, gum tissue and bones. Will you also ask about possibly having an anti-anxiety med pre-treatment?
@jpatterson52 I had been diagnosed with Severe Osteoporosis with Spontaneous Fractures...of which i had two...I was eating chocolate and found my tooth was loose...so had two extractions...I did need the gum cleaning but as you say it was painful and expensive I wish I didn't have gum disease as I always had pride in my teeth😊
@gravity3 the Dentists did resemble Battle Star Gallactica...not a warm atmosphere but I am living in the past....😊

2 ReactionsI feel for you. Maybe you can take headphones and some relaxing music of your choice to your dental appointment. That might help take you away from what's going on. I know this sounds like a simple thing to do but it seems you're very limited in your dental options. Best of luck to you.
Hi @lane73 Oh my gosh, I’m laughing…I saw this post after I just replied to your previous post from yesterday! I’d love to sit down over a cuppa tea with you…I have a feeling there would be more laughter than conversation.
Being claustrophobic is a horrible feeling. I have gotten over the worst of it but still have issues during some situations so I feel your anxiety!
Lying prone through dental treatments can be an issue for many patients. I know you’re not a very assertive person, but you can tell the dentist that he’ll have better success if you could sit up a little more at an angle than lying flat. I’m not a fan either!! Even during cleanings. What can be very helpful is finding mental games to play that keep your focus off what’s happening in your mouth!
One thing that I like to do is visualize a word. Let’s say dentist! Then make as many words as you can starting with the letter D. Then move to letter E and so on. That little game takes a great deal of mental concentration and keeps your mind off the nonsense going on in your mouth. LOL. If you feel at all gaggy, the key is to take deep breaths through your nose!! Yes, it’s a challenge with your mouth open but it’s highly effective.
The news that your dentist did give you antibiotics to start 3 days prior is excellent news! So he is on top of the game!! I really REALLY hope you can relax for the fear of ONJ and get through this with flying colors!! I’m here if you need a back up. LOL. And I loved @gravity3 suggestion to “Let the force be with you!!” We’re standing protective of you, shoulder to shoulder, armed to the teeth…pun intended!
