Transperineal biopsy recovery time?
Howdy fellow prostaters
I just had a transperineal prostate biopsy on Thursday. I liked it much better than the trans-rectal one I had 5 years ago, with spotty local anaesthesia (I remember the sounds "click" followed by "ouch").
I especially loved the general anaesthesia. I wanted to take some home with me, but they said no.
When I came out of the anaesthesia, I was quite chatty, and who knows what silly things I might have said. I do remember one statement I made. I wanted to make a general compliment to the professionalism at Mayo, so I said "This place just reeks of competence!"
It made perfect sense at the time.
But I digress...
I'm peeng more-or-less normal now (i.e. very slow), and I've had no blood in urine or stool. I'm really wondering when I can go back to lifting weights, which has done wonders for my mental attitude this past year or so. One nurse said no exercise for 3 days, one said for a week. Maybe I'll start back in the gym Thursday of next week, and lift 1/2 of the normal weight? 1/3?
Anyone here with experience to share?

@gerryp How many cores were taken? Wondering if a higher number of cores results in an increase of trauma to prostate. I had just 12. But your symptoms sound a little unusual and concerning. Hopefully the next couple days you will see marked improvement.
@mtsenior I had 16 cores taken. 4 in the lesion that was identified by MRI, and then 12 random ones - 6 on each side I believe. Woke up this morning, and looks like someone was murdered in my shorts overnight. No discomfort, or fever, which makes this really strange. I have to call them today. Appreciate your perspective.
@gerryp I had no leaking of blood after my biopsy. This sounds very unusual, so yes good idea to call today.
@gerryp If you haven't already checked in with your urologist, I would do so ASAP. All the bleeding you're experiencing after this many days is not what we've experienced. I've had 2 transperineal biopsies in the last 2 years and had nothing like you're experiencing. Good luck!

1 Reaction@tdgillett Thank you Tommy. I was told to expect blood in my urine - which I definitely have. But not to expect anything like what’s been happening. I’ve noticed that when I first start to pee - it looks like pure blood, and then it kind of clears up to a pinkish tinge. It doesn’t actively leak until I’m sitting or sleeping and I don’t even realize it. The nurse just called me back and I expect to hear back from the doctor soon. Really appreciate your perspective.

1 Reaction@gerryp Hi Gerry, it would help everyone tremendously if you provide an update after visiting the urologist. As stated above several times, what you are experiencing is not to be expected. I had nothing like that. Best to you my friend.
I had a trabsperenial biopsy last May. I experienced no bleeding, no discomfort, no pain. I was told to avoid heavy exercises like lifting weights, squatting etc, for 2 or 3 days: that is all.
Good morning @oldgreenpaint. And the answer is “Friable Prostate”. I went for my 3 appts, they took a urine culture, had me try to void and measured the remaining urine in my bladder. I failed that test, so asked to be able to try to finish peeing. After the second void, I just marginally passed the urine retention test. Docs weren’t happy and wanted to put a catheter in for the weekend and irrigate my bladder. They reluctantly agreed to let me go home without the catheter as long as I would go right to the ER if I couldn’t pee. The blood and difficulty peeing continued until finally just this morning. I bled a little overnight, but so far today, urine looks mostly clear and it’s much easier to pee. I’m 66, and I’ve had BPH for more than 15 years, and I guess BPH makes the prostate more fragile with more vascularity and therefore more subject to random bleeding. And call also be a sign of prostate cancer - so I’m waiting for the biopsy results which will probably come in on Monday. I think the biopsy made my prostate very angry, and the doc said they took 16 cores on an already fragile prostate. Hence all the bleeding. I had this happen one other time about 15 years ago, and my urologist did a cystoscopy and diagnosed a friable prostate. Never had that issue again until last Tuesday following the biopsy. I have another appointment tomorrow to retest everything and am hopeful that I won’t need that damn catheter. Thanks for following up.
@gerryp Thank you Gerry. Most people have no idea of the trauma to the prostate from having 16 cores removed. A biopsy should never be thought of as a benign routine event. Only do it when absolutely necessary. Hoping you heal up well. I have told friends with rising psa scores to get an mri BEFORE a biopsy. If the mri doesn’t detect a suspicious zone, then consider monitoring psa over time before committing to a biopsy.
