Transdermal HRT

Posted by vkmov @vkmov, Oct 19, 2023

Many studies have found that transdermal HRT protects and builds bone without all the horrific side effects of many first-line osteoporosis treatments. The patch bypasses the liver and thereby avoids the risks of breast cancer, strokes, etc. Has anyone explored this or discussed it with their doctors?

I am going to talk to my endocrinologist about HRT. I am 62 (13 years postmenopausal) and have used the creams from BioLabs Pro (Estradiol/Estriol and Progesterone). I would like to go on a transdermal patch or other low dose treatment. Can members share the products they are on and doses?

@amriv I am on 14 mcg Menostar and 50 mg progesterone cream and vaginal estradiol. I am 75 and 20 years past menopause. I had to go to a functional physician to get the Menostar and progesterone cream. My urologist prescribed the the vaginal estradiol.

Dr. Sue,
I'll be starting on Menostar patches this week, probably tomorrow! I've read that the schedule is one Menostar patch applied weekly for 3 weeks followed by 1 week off. Repeat. Is this the same schedule you are using for Menostar?

I'm also prescribed 100 mg of oral progesterone - one capsule every day for 14 days to begin after the first one month of Menostar. I'm not actually sure if the progesterone should be started after 3 weeks or 4 weeks since the Menostar is only used for 3 weeks followed by one week off. No additional progesterone until the next 3 months of Menostar to be applied as above.

Can you tell me more about the 50 mg of progesterone cream you're taking? I'm wondering if this is applied every day or otherwise? Where is this applied? Is this compounded?
Thank you!

@amriv , I’m same age as you. I’m on transdermal patch .05 mg, with 100 mg progesterone every third night. I’ve been on HRT for about 1.5 years and am still slowly modifying the dose to find the right combo of estradiol and progesterone.

My primary care doc believes in testing blood serum levels, which is why we keep modifying. Some docs don’t believe in testing (like my endocrinologist). If we weren’t testing, I would have settled on a different dose long ago— probably .0375 patch with 100 progesterone nightly.

Good luck. HRT has been a good experience for me.

I've been thinking more about my directions as a brand new patch user. Thought I'd ask for others' experiences...
Are you applying the patch once a week? Do you apply on a schedule of 3 weeks with patch and one without? Are you taking the progesterone every 3rd day continuously?
Thank you!

I have been taking Forteo for about 1 year. Can one take HRT at the same time? I should have asked about this before I started taking Forteo. I had a full hysterectomy a year ago as well. I am 62 and probably went through menopause at 57. I don't have the night sweats and am not tired, but have no emotion and anxious.

@tillymack, yes, I'm applying the estradiol patch once a week, continuously. I've done that consistently since I started HRT a year and a half ago.

Until about 6 weeks ago, I'd done progesterone continuously, too. But my progesterone levels have been pretty high (which gives me breast tenderness). 6weeks ago, my provider suggested I try cyclical instead. I tried one cycle of 14 days off and 14 days on. Didn't like it! Started spotting and didn't sleep as well, so I've switched back to continuous for progesterone... 100 mg every 3rd night.

Good luck as you work your way through this. Lots of tweaking to get it right (if your provider tests levels...) Overall, I am really pleased with HRT and glad I'm on it.

I am now taking the compounded progesterone cream every day along with the Menostar patch. My original script was for micronized progesterone in a capsule, but I had difficulty with that. As a matter of fact, I was on the patch alone for 5 months. The Menostar insert recommends progesterone just twice a year for 2 weeks. It sounds like you are trying to follow a normal menstrual cycle. I hope the Menostar works for you.

This sounds most like a Prolia reaction. Prolia has listed side effects of necrosis of the jaw and osteonecrosis of the external auditory canal (bone death/breakdown in the ear canal.
This may be exaceberated by steriod use, but the Prolia really seems to be the problem.
I am so sorry that you are going thru this reaction. I was lucky I just couldn't take the osteoporosis drugs which we prescribed. My friend who was fracturing and stopped after she began strontium citrate says that we won the bone lottery.
You might try strontium citrate; it is an earth mineral found in most ground water and food; we just take it at therapeutic dose level of 680mg nightly.

@kathleen1314 As I understand it, you are the #1 cheerleader for strontium on Mayo Connect's osteroporosis chat. Yet you also take hrt.

HRT appears to have generally accepted osteroporosis treatment success. So how do you know that you won the 'bone lottery' with strontium if you are being treated with both HRT and strontium?

And if it possible for you to respond without overwhelming cites generated by AI, that would be most helpful.

@gravity3 I am just starting to speak to a clinic about the pellets. I am 68 and fear that the pellets might not work for me, as no way to adjust dose once it's in. Did you have any side effects or need adjustments?

@blueberre
Use the topical cream instead.
