Sudden decline in eGFR in the elderly

92 yr old drop in eGFR by 9 in less than a month currently sits at 35. Pacemaker procedure 4 weeks ago all vitals now are stable all other bloodwork is normal Increased memory loss, feeling unwell other symptoms of dizziness, stomach bloating, sweating off, heavier breathing on and off consitantly feeling unwell. CT scan in 3 weeks ultrasound came back clear. Trying to figure out if this is CKD, normal aging process, psychological issues or all of the above. Also if a CT is dangerous for her with or without contrast. Has not been referred to a neuphologist yet.

If you don’t mind me asking, could you share a little more about how your loved one has been day-to-day since getting the pacemaker? Have the dizziness, heavier breathing, and bloating been constant or do they come and go? Have there been any changes in eating, drinking, or sleeping?

Symptom's come and go part of me thinks it could be normal aging process but not sure that’s why Dr has asked for CT scan and I’m very worried about any contract because of low kidney function. Some appetite loss but nothing to be concerned about again it’s sporactic. She was dizzy the other day again.

I believe she should definitely see a nephrologist with her GFR. Although there is no cure for CKD, there are ways to slow the decline, and diet plays a big part. Many online sources of information. Best of luck.

The sudden drop in eGFR could be attributed to the recent start of the pacemaker. eGFR can easily change when other conditions change. It takes awhile to get a sense of the overall trend. Her other symptoms could be indicative of CKD. If it were me I would ask why the CT scan was ordered. (What are they looking for &/or hoping to rule out?) I would also ask that provider about CKD, for a referral to nephrology and about your concerns regarding contrast dye. I’m assuming your loved one has given authorization for you to ask questions and get information on her behalf?

Yes I am her personal directive(dtr) CT is scheduled for Tues and I believe it’s to rule out any “hot spots” because of her upper abdominal issues with bloating. I’ve requested low dose or no contrast because of her kidney function it has went down considerably in the last two years and by nine within a two weeks period to 35 eGFR

I would suggest swtiching 100% veg diet ( if you haven't) , so that the existing nephrons are not harmed.. Nephrons are the filters which decide the Kidney GFR. ( In my view GFR over 30 & above is acceptable but it needs to be stabilized.
