Stage 4 Prostate Cancer and Hormone Therapy quit working

I have stage 4 PC and am doing Lupton injections/3 monthly intervals plus radiation treatment, so far having received 8/20 doses.
One thing I think about is quality of life and I would like to hear what others have to say on the quality of life question.

I don’t see you mentioning having Pluvicto treatments. That is usually the next step. It doesn’t work at all for 1/3 of people, but it is very successful for 1/3.

Not sure what you mean by big prednisone. Not sure if you have been on Zytiga which requires you take 5 mg of prednisone. That might work for you, it might not, you need to ask your oncologist if it makes sense.

I have stage IV prostate cancer metastasized throughout my bone structure. I am going on 5 years of various treatments and am coming to the end. They have run out of things to try.

There are several treatment options available to you yet. You are still at the early stages. Depending on what all is involved in your cancer and where it is your oncologist may recommend targeted radiation treatments. I generally have had decent success with that. I also have had the six course treatment of Pluvicto radiation infusion treatments, again with good success. As of now they have run out of treatments. The thing is, they all work for a while and then they stop working. Hopefully you have several years to go yet and multiple other avenues before you reach the end of life stage that I am at with stage IV prostate cancer which has metastasized throughout my bones and lymph nodes.

I'm sorry to hear about your new challenges. You've been carrying on the fight for 7 years already, and the fact that you're feeling great at 80 gives me hope that you have many, many more years of joyful life in you.

I can't remember for sure, but I think you're the one who mentioned that Princess Margaret Hospital in Toronto is the world's 10th highest ranked cancer centre, behind Mayo Rochester (4th place) but ahead of the Cleveland Clinic (17th place) and the other Mayo satellite clinics. Are you getting treatment there? If so, then you can be pretty confident that the options they give you for "what's next?" will be the best and latest. But really, any major Canadian cancer centre associated with a university medical school will be great, because practitioners/researchers move back and forth among the top cancer treatment clinics (my new oncologist just came back home to Canada from Harvard med school), and they carry their knowledge around with them.

If zolodex is working, there is no reason to do BAT. You do it when no other drug keep your PSA down.

You definitely should consider Lutetium. It’s very successful in about 1/3 of the patients somewhat successful in 1/3 and does nothing at all and 1/3. of course if your PSA is under control, you don’t want to do that yet.

You should get a somatic genetic test before doing lutetium. It does not work well if you have RB1 or PTEN genetic anomalies. a somatic test can tell whether or not you have those. There are many genetic anomalies that can pop up after having prostate cancer.

Xgeva is only to prevent bone loss, which happens with the cancer drugs you take for prostate cancer.

It is unlikely that Darolutamide will work if Enzalutamide (xtandi) stopped working. You could try it, ask your oncologist

If you have not been on Zytiga that may be a better choice. It is usually used before one of the lutamides but it probably would work after. Ask your Oncologist.

Another option would be to try BAT? This is where you Get testosterone injections for a few months and it hopefully resets your ability to have a drug like Xtandi work again. Doesn’t work with everyone.

I appreciate it when you and others share their stories about the next steps with stage-4 PCa treatment. I'm still holding at castrate-sensitive for now, and I know from my oncology team that there are many options waiting for me if/when my PCa becomes castrate resistant, but hearing personal experiences is worth far more than sterile knowledge or articles.

Am taking Xofigo ( infusion ) every four weeks for six doses—had first treatment couple of weeks ago. Tolerated well—-continue with Nupron—no numbers yet on effectiveness—will be happy to share if you wish
