Should I Be Concerned? Blood test suggestive of MGUS

A year ago, I was sent to a nephrologist for hematuria, creatinine clearance by Cockcroft-Gault of 52, creatinine 1.24. He did some blood test that showed free kappa 30.7 (3.3-19.4), free lambda 18.1 (5.7-26.3) and ratio 1.7 (.26-1.65) interpretation states suggestive of MGUS.
I have history of peripheral neuropathy, night sweats (? Menopause causing), osteoporosis, (fractured wrist after fall requiring plate and 8 screws) have had a spinal fusion due to nerve compression, always fatigued and walking short distances (mowing lawn 30 minutes) tired, weak legs (feel like rubber bands) after just picking up sticks in the yard, and recently bruising very easily (don’t know how getting most of them) and cold sores that haven’t had since being on daily meds. I also have history of lymphocytic colitis, always thirsty so urinate frequently, nocturnal 2-3x a night, not losing weight though, short of breath after activity but likely out of shape. I am a 61 yo white female, BMI 28.2. Several ortho surgeries for nerve compression in foot, ankle, knee, elbow, lumbar/sacral area and shoulders, hip replacement. Wondering if should make an appt with ?.

I would definitely see a hematologist oncologist. While your kappa and K/L ratio is not alarming you still should get a full work up. What was your M-Spike? Did they have you do a 24/hr urine ?

The electrophoresis interpretation evidently didn’t find a M spike but said to complete light chains if clinically indicated. No 24 hour urine.

A hematologist ASAP go have a bone marrow test samples drawn. They can determine both your cancer and how active by doing histology on the marrow and other tests.

Your ratio is normal and with no abnormal protein I don’t think you have MGUS or anything alarming. My oncologist advised me the light chain ratio is the important number not the individual numbers as they are likely do to inflammation
