PSA up then down

I am 60. PSA Dec 2023 was .9. Went to 2.0 in Dec 2024. Retested a month later at 1.4. PA urologist says MRI now. First urologist i went to didn’t even do DRE or think i needed another PSA test for another 3 months. This is all so confusing. Any thoughts?

PSA is not back to baseline yet but is 1.4 right now. I will ask about PSE test. Yeah pi rad 4 freaked me out. I also don’t understand it. When I called about biopsy appt the nurse sounded confused about it too as did my PCP. Getting second opinion.

My last 5 Total PSA scores are 8.1, 7.8, 5.4, 4.4, 7.1 over an 8 month period. Free PSA % also increasing from 13.5% to 16.6%. MRI was pirads 3 with an enlarged prostate of 95ml. I have another MRI scheduled for October. It’s all nerve wracking.

Did he have any family history or prostate related symptom?

An enlarged prostate can be the cause for a high PSA. A Friend of ours had a PSA of 50 and a huge prostate, Yet after multiple biopsies, he’s never had prostate cancer.

Has your doctor Discussed BPH and maybe getting on an antibiotic to reduce the size of your prostate?

I appreciate the reply. I think that is the next step. I’ve read that Finasteride is the standard for BPH. I have a second MRI scheduled for 10/6. Then depending on the results either another PSA or Biopsy if there is something more suspicious…….its a roller coaster journey.
